摘要
目的利用非接触式自动角膜内皮计对较大样本中老年人角膜内皮细胞和角膜中央厚度进行分析,获得其结构参数的生理正常值范围。方法对670例中老年白内障患者术前角膜内皮检查资料进行回顾性分析。206例采用Topcon内皮计,464例采用Humphrey内皮计。相关结构参数包括内皮细胞密度,内皮细胞平均面积,最大和最小面积,面积标准差和变异系数,六边形细胞所占比例,以及中央角膜厚度。结果统计资料平均年龄(69±9)岁(41~94岁)。Topcon内皮计和Humphrey内皮计单纯白内障患者平均角膜内皮细胞密度分别为(2683±417)个/mm2和(2696±404)个/mm2,均有随年龄增长而下降的趋势,60岁上下出现较显著性差异。内皮细胞平均面积分别为(381±65)μm2和(381±75)μm2。Topcon内皮计测得平均中央角膜厚度为(0.541±0.037)mm。Humphrey内皮计测得六边形细胞所占比例为(53±10)%。白内障合并青光眼患者65例,其平均细胞密度显著低于单纯白内障患者(p<0.01),平均细胞面积显著增大(p<0.01)。结论中老年人眼角膜内皮细胞数量和大小仍随年龄变化而改变,60岁上下出现较显著性差异。了解不同年龄段人群的角膜内皮参数正常值有助于为各类眼病治疗或内眼手术提供安全保障。Topcon内皮计和Humphrey内皮计的测量结果具有可比性。
Object To analyze corneal endothelial cell counting and central corneal thickness of middle-aged and senile people using non-contact automatic specular microscope,provide physiological normal range of some structural parameters. Method We analyzed pre-operative specular microscope data of 670 cases of middle aged and senile cataract eyes retrospectively,206 cases using Topcon specular microscope and 464 cases using Humphrey specular microscope. Sturctural parameters include corneal endothelial cell counting, average area of endothelial ceUs,maximum and minimum cell areas,standard deviation and coefficient variation of cell area,proportion of hexagon cell,and central corneal thickness. Result Average age for our statistic data is (69 ± 9) years old (41 -94). The average corneal endothelial cell density of single cataract eye is (2683 ± 417)/mm^2 and (2696 ± 404)/mm^2 respectively for TOPCON and HUMPHREY.They both showed decreasing tendency as the age increased,and showed significant difference above and below 60years old.The average cell area is (381 ± 65)μm^2 and (381 ± 75)μm^2 respectively, we got average central corneal thickness is (0.541 ± 0.037) mm by Topcon specular microscope. And We got the proportion of hexagon cell is (53 ± 10) % using Humphrey specular microscope.There are 65 cases complicated with glaucoma. The average cell density (2435 ± 650)/mm^2 is significantly lower than that of simple cataract eyes(p〈0.01). Conclusion Corneal endothelial counting is changing with age among middle-aged and senile people.There exists significant difference above and below 60 years old. Acknowledge of normal range of corneal endothelial cell counting for different age phases can provide us more security for therapy on corneal or intraocular operations .The measuring result of Topcon and Humphrey specular microscopy is comparable
