急性冠脉综合症患者平均血小板体积和血浆纤维蛋白原的变化及临床意义 被引量:8

摘要 目的探讨急性冠脉综合症(ACS)患者平均血小板体积(MPV)和血浆纤维蛋白原(Fg)的变化及临床意义。方法将315例经冠脉造影确诊的ACS患者(其中不稳定型心绞痛(UAP)患者146例、急性心肌梗死(AMI)患者169例(其中非ST段抬高型心肌梗死患者67例和ST段抬高型心肌梗死患者122例)和96例稳定型心绞痛(SAP)患者,将它们分为UAP组、AMI组和SAP组,同时选择100例健康人作为对照组,测定各组人群MPV、Fg、血清生化指标、血常规其他参数和血清心肌钙蛋白I(cTnI)等。结果 (1)AMI组MPV(11.87±1.173 fl)高于UAP组(11.36±1.103fl)、SAP组(10.60±0.869fl)和对照组(10.48±1.125fl),差异有统计学意义(P<0.01);UAP组MPV高于SAP组和对照组,差异有统计学意义(P<0.01)。SAP组MPV和对照组之间无显著差异(P>0.05);(2)AMI组Fg(3.75±1.339g/L)高于UAP组(3.08±0.750g/L)、SAP组(2.78±0.638g/L)和对照组(2.69±0.510g/L),差异有统计学意义(P<0.01)。UAP组Fg高于SAP组和对照组,差异有统计学意义(P<0.01)。SAP组Fg和对照组之间无显著差异(P>0.05);(3)双变量相关分析显示ACS患者MPV和Fg均与PDW(r=0.942,P=0.000;r=0.122,P=0.031)和CK(r=0.119,P=0.032;r=0.219,P=0.000)呈正相关,与Hb(r=-0.189,P=0.001;r=-0.147,P=0.009)呈负相关;MPV和Fg之间呈正相关(r=0.118,P=0.036)。结论 ACS患者MPV和Fg均显著升高,不同危险分层的ACS患者MPV和Fg水平存在显著差异,测定MPV和Fg对于ACS患者的危险分层有一定的价值。
