摘要
目的探索建立与经济发展水平相适应的城乡居民基本医疗保险筹资政策。方法运用辩证唯物主义观点,分析济南市城乡居民基本医疗保险筹资政策存在的问题及其成因。结果城乡居民基本医疗保险统一定额的个人缴费机制缺乏公平性、可持续性,呈现不规范性、难预期性,加重了逆向补偿和逆向选择,筹资和保障水平偏低;各级财政之间,特别是县域财政之间负担失衡。结论整合后的居民医保没有从根本上改善筹资的公平性,应建立随各主体方收入水平增长而增长的筹资机制。
OBJECTIVE Explore a compatible natural fund-raising mechanism with the level of economic development to Basic Medi- cal Insurance for Urban and Rural Residents (RBMI). METHODS Materialistic view was adopted to analyze the financing policy of RBMI in Ji'nan. RESULTS Unified quota individual payment mechanism was lack of equity, sustainability, standardization and difficult to predict; increased the reverse compensation and adverse selection; leads to low financing and corresponding security level. The economic burden was out of balance among all levels of finance, especially among county' s finance. CONCLUSION After the integration of RBMI, the fairness of its financing was not fundamentally improved. It should establish a kind of RBMI financing mechanism which changes with the level of income growth contributed by each party.
