摘要
目的探讨高血压致慢性心力衰竭患者T细胞及中性粒细胞亲环素A和亲环素B的表达特点。方法选择原发性高血压2~3级患者150例,根据合并疾病情况分为原发性高血压无心脏疾病患者50例(A组)、原发性高血压并高血压性心脏病,心功能(NYHA)Ⅰ~Ⅱ级患者50例(B组),原发性高血压并高血压性心脏病心功能Ⅲ~Ⅳ级患者50例(C组),另选择健康体检者50例(对照组),比较各组心脏彩色超声,炎性因子及抗氧化能力,同时采用RT-PCR和Western blot方法检测血清T细胞及中性粒细胞中亲环素A和亲环素B mRNA及蛋白表达水平。结果与对照组比较,A组、B组、C组血管紧张素Ⅱ、内皮素1、肿瘤坏死因子α以及白细胞介素1β明显升高,总抗氧化能力明显降低(P〈0.05);与A组比较,B组和C组血管紧张素Ⅱ、内皮素1、肿瘤坏死因子α以及白细胞介素1β明显升高,总抗氧化能力明显降低(P〈0.05)。与对照组比较,A组、B组及C组血清T细胞以及中性粒细胞亲环素A和亲环素B mRNA明显升高(1.12±0.34、1.28±0.38、1.43±0.36 vs 0.78±0.12,0.80±0.21、0.87±0.18、0.86±0.17 vs 0.47±0.16;1.42±0.36、1.56±0.34、1.77±0.35 vs 0.84±0.31,1.08±0.25、1.17±0.28、1.50±0.37 vs 0.72±0.19),同时蛋白表达也明显升高,差异有统计学意义(P〈0.05)。结论高血压患者应尽早调节血清亲环素A和亲环素B水平,可能对降低高血压引起的心功能损伤有一定的意义。
Objective To study the expression of cyclophilins A and t3 in T cells and neutrophils of patients with hypertension-induced chronic heart failure (CHF). Methods One hundred and fifty primary hypertension patients were divided into primary hypertension group without heart disease (group A,n=50),primary hypertension group with heart disease (group B,n= 50),and primary hypertension group with heart function damage (group C,n=50), another 50 healthy persons served as a control group. Their color ultrasonographic findings,inflammation and anti-oxidation conditions were compared. Expressions of cyclophilins A and B in their T cells and neutrophils were detected by RT-PCR and Western blot,respectively. Results The serum angiotensin Ⅱ ,endothelin-l,TNF-α and IL-1β levels were significantly higher and the total antioxidant level was significantly lower in groups B and C than in group A (P〈0.05). The expression levels of angio- tensin II , endothelin-1,TNF-α and IL-1β were significantly higher and the total antioxidant level was significantly lower in T cells and neutrophils of groups A, B and C than in those of control group (P〈0.05). Conclusion Early regulation of serum cyclophilins A and B levels plays an im- portant role in reducing hypertension-induced cardiac function damage.
