孕妇泌尿生殖道无乳链球菌感染的耐药性分析 被引量:22

摘要 目的分析孕妇泌尿生殖道无乳链球菌的耐药性,为临床感染的预防、合理用药提供参考依据。方法收集医院2009年2月-2013年11月门诊及住院3 000例孕产妇的临床资料,培养鉴定阴道分泌物、宫腔内容物、中段尿以及羊水培养中的细菌,并对分离的无乳链球菌进行药敏试验,统计分析数据采用WHONET 5.5软件进行。结果分离出无乳链球菌281株占9.37%,其中阴道分泌物检出率居首位,阳性率为12.61%,羊水检出率最低,阳性率为0.93%;所有无乳链球菌对利奈唑胺、头孢唑林、万古霉素敏感率为100.00%;对头孢曲松、青霉素无耐药株,但有中介株,中介率分别为2.85%、1.78%;对红霉素、克林霉素、左氧氟沙星耐药率分别为60.50%、55.87%、47.69%。结论对孕妇泌尿生殖道进行无乳链球菌药敏试验,不仅能有效地预防该菌感染,还可以为临床提供治疗及指导依据,进一步预防其危害性。 OBJECTIVE To analyze the drug resistance of the Streptococcus agalactiae causing urogenital tract in‐fections in pregnant women so as to provide guidance for prevention of clinical infection and reasonable use of anti‐biotics .METHODS The clinical data were collected from 3 000 pregnant women who were treated in the outpatient department or hospitalized from Feb 2009 to Nov 2013 .The vaginal secretions ,uterine cavity contents ,midstream urine ,and amniotic fluid were cultured for bacteria ,the drug susceptibility testing of the isolated S . agalactiae strains was carried out ,and the statistical analysis of data was performed with the use of WHONET 5 .5 software . RESULTS Totally 281 (9 .37% ) strains of S .agalactiae were isolated ,of which 12 .61% were isolated from the vaginal secretions and 0 .93% were isolated from the amniotic fluid .The drug susceptibility rates of all the S . agalactiae strains to linezolid ,cefazolin ,and vancomycin were 100 .00% ;no strains resistant to ceftriaxone or penicillin were found ,but there were the strains intermediate to ceftriaxone and penicillin ,with the intermediate rates of 2 .85% and 1 .78% ,respectively .The drug resistance rates to erythromycin ,clindamycin ,and levofloxa‐cin were 60 .50% ,55 .87% ,and 47 .69% ,respectively .CONCLUSION It is an effective way to conduct the drug susceptibility testing for the S .agalactiae strains isolated from the urogenital tracts of the pregnant women so as to prevent the infection ,provide guidance for clinical treatment ,and further avoid the hazard .
