摘要
目的观察临床护理路径(clinical pathway,CP)在心脏瓣膜置换围术期患者护理中的应用效果。方法便利抽样法选择选择2010年1月至2011年10月在深圳市孙逸仙心血管医院行心脏瓣膜置换术的158例患者为对照组,未实行临床路径。同法选择2011年11月至2013年12月在深圳市孙逸仙心血管医院行心脏瓣膜置换术的134例患者为观察组,其围术期护理按照临床路径制定好的医、护、患临床路径表实施每日诊疗、护理、术后康复指导工作。比较两组患者的平均住院日、住院费用、ICU停留时间、并发症的发生率、患者满意度等指标。结果观察组患者平均住院日、ICU停留时间、平均住院费用分别为(19.18±2.04)d、(28.24±6.83)h、(57426.31±4877.25)元,均低于对照组,差异均有统计学意义(P<0.05或P<0.01)。两组患者并发症的总发生率及两组患者的满意度差异均有统计学意义(均P<0.05)。结论临床护理路径的实施,能缩短患者平均住院日和ICU停留时间,降低住院费用,降低手术后并发症的发生率,提高护理质量和患者的满意度。
Objective To observe the effects of clinical pathway （CP） in perioperative nursing of cardiac valve replacement.Methods By convenience sampling, 158 cases underwent cardiac valve replacement from January 2010 to October 2011 were selected as control group and didn＇t received CP.The other 134 patients from November 2011 to December 2013 were selected as observation group and received CP during periop- erative period.Daily treatment was based on planned medical, nursing, patient clinical path table. Average hospitalization day, hospitalization expenses, ICU stay time, the incidence of complications, and patient sat- isfaction index of two groups were compared.Results The hospitalization day,ICU stay time and hospitali- zation expenses of experimental group were 19.18 ± 2.04,28.24 ± 6.83,57426.31 ± 4877.25, respectively, which were lower than that of control group （P〈0.05 or P〈0.01）.The difference was statistically signifi- cant in the overall rate of incidences of complications and patient satisfaction index（all P〈0.05）.Conelusion The implementation of clinical pathway can shorten patient ′s average hospitalization day and ICU stay time, reduce the hospitalization expenses and the incidence of complications after the operation, and can also improve the quality of nursing and patient＇s satisfaction level.
