PCI治疗对急性心肌梗死患者CD^(34+)单个核细胞及血管内皮功能的影响

摘要 目的研究经皮冠状动脉内介入治疗(PCI)对急性心肌梗死(AMI)患者CD34+单个核细胞及血管内皮功能的影响。方法采用流式细胞分析法对40例PCI治疗的AMI患者、40例非PCI治疗的AMI患者及20例无心脏病的对照组的CD34+单个核细胞百分率进行动态监测。内皮功能采用动脉流量介导舒张(FMD)指标评价。结果 AMI患者(PCI组和非PCI组)CD34+单个核细胞百分率在AMI后第2~3天开始升高,在第3天后持续升高,在第7天达到最高峰后持续下降,第30天CD34+单个核细胞百分率仍高于第2天。在第90天时,已基本回降至第2天水平。在第120天时,基本恢复正常水平。PCI组的CD34+单个核细胞胞百分率明显高于非PCI组,差异有统计学意义(χ2=5.44,P<0.05)。PCI组的CD34+单个核细胞数量在PCI术后第1天与术前比较开始升高,差异有统计学意义(χ2=7.30,P<0.05),术后第2天开始明显升高,差异有统计学意义(χ2=6.74,P<0.05)。AMI发病1周时,PCI组和非PCI组的FDM均低于对照组,差异均有统计学意义(t分别=3.19、8.51,P均<0.05)。结论 PCI能促进AMI后CD34+单个核细胞的表达,能改善AMI患者的血管内皮功能。 Objective To evaluate the influence of percutaneous coronary intervention on CD34 +mononuclear cells and endothelial function in patients with acute myocardial infarction(AMI). Methods Peripheral blood mononuclear cell percentages of CD34+were dynamic monitored from 40 patients in PCI group,40 patients in Non-PCI group and 20 patients in control group without heart disease. The endothelial function was evaluated by the artery flow-mediated relaxation(FMD).Results The mononuclear cell percentages of CD34+in PCI and Non-PCI groups increased at 2ndor 3rdday after occurring AMI until the 7thday. And then, the mononuclear cell percentage of CD34 +was decreasing until the 30 thday which was higher than the mononuclear cell percentage of CD34+at 2ndday. At the 90 thday, the mononuclear cell percentage of CD34+was mostly returned to the level of the 2ndday, and the mononuclear cell percentage was basically normal until to the 120 th day. The mononuclear cell percentage of CD34 +in PCI group was significantly higher than Non-PCI group(χ2=5.44,P〈0.05). In PCI group, compared with before operation, the mononuclear cell percentage of CD34+at first day after operation was significantly increased(χ2=7.30, P〈0.05) as well as the 2nd day(χ2=6.74,P〈0.05). When AMI for one week, the FDM of PCI and Non-PCI groups were significantly lower than the control group(t=3.19,8.51,P〈0.05). Conclusion PCI can promote the expression of CD34+mononuclear cells and improve endothelial function in patients with AMI.
