摘要
目的探讨血清胱抑素C(cystatin,Cys C)和尿微量白蛋白(micro-albumin,m ALB)在过敏性紫癜(henoch-schonlein purpura,HSP)患儿早期肾功能损伤中的临床应用价值。方法收集2014年1月至2014年12月我院皮肤科120例HSP患儿病历资料,根据其肾功能划分为肾功能正常组(60例)和肾功能异常组(60例),同时选取60例健康体检儿童为对照组。检测受试儿童血清Cys C和尿m ALB水平,对检测结果进行统计学分析。结果对照组、肾功能正常HSP组、肾功能异常HSP组血清Cys C和尿m ALB检测结果差异均有统计学意义(F=7.13,F=8.54,P均<0.05)。肾功能正常HSP组和肾功能异常HSP组血清Cys C和尿m ALB检测结果均高于对照组,且差异均有统计学意义(P均<0.05);肾功能异常HSP组血清Cys C和尿m ALB检测结果高于肾功能正常HSP组,且差异均有统计学意义(P均<0.05)。HSP组血清Cys C、尿m ALB及二者联合检测阳性率均高于对照组,且差异均有统计学意义(P均<0.05);HSP组血清Cys C和尿m ALB联合检测的阳性率高于两指标单独检测的阳性率,且差异均有统计学意义(P均<0.05)。结论血清Cys C和尿m ALB可作为预测HSP患儿早期肾损伤的敏感指标。
Objective To explore the clinical value of serum cystatin C(Cys C) and urinary micro-albumin(m ALB) detection in the diagnosis of early renal function damage in the children with henoch-schonlein purpura(HSP). Methods 120 cases of HSP children from January 2014 to December 2014 in our hospital department of dermatology were collected. They were divided into normal renal function group(60 cases) and renal dysfunction group(60 cases) according to their renal function. In the same time, 60 healthy children were chosen as control group. The levels of serum Cys C and urine m ALB of all the children were detected and the results were analyzed statistically. Results There were statistical significance in the differences of serum Cys C and urine m ALB levels among control group, normal renal function group and renal dysfunction group(F =7.13, F= 8.54, Pall〈 0.05). The serum Cys C and urine m ALB levels in normal renal function group and renal dysfunction group were all higher than that of control group, and the differences all had statistical significance(Pall〈 0.05). The serum Cys C and urine m ALB levels in renal dysfunction group were all higher than that of normal renal function group, and the differences all had statistical significance(Pall〈 0.05). The positive rates of serum Cys C, urine m ALB and combine detection in HSP group were all higher than that of control group,and the differences all had statistical significance(Pall〈 0.05). The positive rates of serum Cys C and urine m ALB were all lower than that of combine detection, and the differences all had statistical significance(Pall〈0.05). Conclusion The serum Cys C and urine m ALB can be used as the index for the diagnosis of early renal function in the children with HSP.
