摘要
目的探讨儿童过敏性紫癜肾脏受累时纤溶系统变化特点及其与肾损伤的关系。方法对52例紫癜性肾炎(HSPN)、75例普通型过敏性紫癜(HSP)和25例正常儿童的血浆纤维蛋白原(FIB)、D-二聚体(D-D)、纤维蛋白降解产物(FDP)及24 h尿蛋白排泄量进行检测并进行相关分析。结果 HSPN组和HSP组FIB、D-D水平均明显高于正常对照组(P<0.01);HSPN组FDP和24 h尿蛋白排泄量高于正常对照组(P<0.01);HSPN组D-D和24 h尿蛋白排泄量高于HSP组(P<0.01)。HSPN组血浆D-D、FDP与24 h尿蛋白排泄量呈显著正相关(r=0.475,P<0.01;r=0.529,P<0.01),FIB与24 h尿蛋白排泄量虽呈正相关,但无统计学意义(r=0.193,P>0.05)。结论 HSPN患儿疾病活动期存在凝血、纤溶系统异常,继发性纤溶可能参与本病的肾损伤过程。
Objective To investigate the changes of fibrinolysis system in Henoch-Schnlein purpura nephritis(HSPN) in children and the relationship with kidney injury.Methods Plasma fibrinogen(FIB),fibrin D-dimer(D-D),fibrin degradation products(FDP) and 24-hour urinary protein excretion were investigated and the correlation were analyzed in 52 children with HSPN.75 children with no abnormal Henoch-Schnlein purpura(HSP) were also observed and 25 normal children as controls.Results The plasma FIB and D-D level in the HSPN group and the HSP group were significantly higher than that in the control group;The plasma FDP in the HSPN group were all significantly higher than that in the control group(P0.01);Plasma D-D in the HSPN group was significantly higher than that in the HSP group(P0.01).There was significantly positive correlation between the levels of plasma D-D and 24-hour urinary protein excretion(P0.01),FDP and 24-hour urinary protein excretion(P0.01).There was positive correlation between FIB and 24-hour urinary protein excretion,but no statistical significance(r=0.193,P0.05).Conclusion There were the abnormality of blood coagulation and fibrinolysis in acute stage of HSPN.The secondary fibrinolysis are probably involved in the process of kidney injury in HSPN children.
