摘要
目的研究结肠镜检查过程中体位的改变对肠道息肉检出率(polyp detection rate,PDR)的影响。方法选取2013年6月-2014年1月于北京市昌平区中医医院进行结肠镜检查的140例患者。当进镜至回盲部准备退镜检查时,所有患者均先后采用两种检查体位,即左侧卧位和动态体位(盲肠、升结肠和结肠肝曲处采取左侧卧位,横结肠采取仰卧位,结肠脾曲、降结肠和乙状结肠采取右侧卧位),按结肠分段从盲肠至乙状结肠逐步进行。记录检出息肉的大小、形状、位置、病理结果。计算出不同结肠区域在不同体位时的息肉检出率。结果动态体位下检出的息肉数由左侧卧位下的148枚增加至178枚(χ2=32.76,P<0.05),因体位变换额外检出的30枚息肉大小均值为3.2 mm,动态体位下检查息肉阳性的患者由左侧卧位下的69例增加至92例(χ2=26.29,P<0.05)。结论结肠镜检查过程中体位的动态变化是提高息肉检出率有效而简便的方法。
Objective To investigate the influence of polyp detection rate( PDR) about different body positions during the colonoscopy. Methods One hundred and forty patients who had the colonoscopic examination from Jun. 2013 to Jan. 2014 were selected. All patients were successively adopted two kinds of inspection posture when the colonoscope arrived to ileocecus and ready to have a endoscopic examination,namely the left lateral decubitus position and dynamic positon( left lateral decubitus positon for the cecum,ascending colon and hepatic flexure; supine for transverse colon;and right lateral decubitus position for splenic flexure,descending and sigmoid colon),and the procedure was performed segment by segment from the cecum to sigmoid colon. Results The number of polyps detected by dynamic posture increased to 178 from the 148 of left lateral decubidus position( χ^2= 32. 76,P〈0. 05). The median size of the 30 polyps during additional position changes was 3. 2 mm. The number of patients who had at least one polyp by dynamic posture increased to 92 cases from 69 cases of the left lateral decubidus position( χ^2= 26. 29,P〈0. 05). Conclusion Dynamic changes of body positions in the process of colonoscopy is an effective and convenient method to improve PDR.
