摘要
目的：探讨米氮平联合小剂量奥氮平治疗精神病性抑郁症的疗效和安全性。方法将77例精神病性抑郁症患者随机分为两组，均晨口服米氮平治疗，观察组联合小剂量奥氮平治疗，观察3个月。治疗前后采用抑郁自评量表评定临床疗效，副反应量表评定不良反应。结果治疗2周末起两组抑郁自评量表评分均较治疗前显著下降（P＜0．01），治疗各时点观察组评分显著低于对照组（ P＜0．05或0．01），治疗3个月观察组显效率为76．9％、总有效率为92．3％，对照组分别为52．6％、73．7％，观察组显效率、总有效率显著高于对照组（ P＜0．05）。两组不良反应均轻微，不良反应发生率比较差异无显著性（P＞0．05）。结论米氮平联合小剂量奥氮平治疗精神病性抑郁症具有增效作用，起效快，疗效显著，安全性高。
Objective To explore the efficacy and safety of mirtazapine combined with small dose olanzap‐ine in the treatment of psychotic depression .Methods Seventy‐seven psychotic depression patients were randomly divided into two groups ,both groups took orally mirtazapine in the morning ,and observation group was plus small dose olanzapine for 3 months .Efficacies were assessed with the Self‐rating Depres‐sion Scale （SDS） and adverse reactions with the Treatment Emergent Symptom Scale （TESS） .Results Since the end of the 2nd week the SDS scores of both groups lowered more significantly compared with pre‐treatment （P〈0 .01） ,so did those at each time‐point in observation than in control group （P〈0 .05 or 0 .01） ,at the end of the 3rd month obvious and total effective rate were respectively 76 .9% and 92 .3% in observation and 52 .6% and 73 .7% in control group ,the former were significantly higher than the latter （P〈0 .05） .Adverse reactions of both groups were mild ,their incidences had no significant difference （P〉0 .05） .Conclusion Mirtazapine plus small dose olanzapine has a synergism ,takes effect rapidly , and has an evident effect and higher safety in the treatment of psychotic depression .
