摘要
目的了解肇庆市社区获得性肺炎患儿肺炎链球菌的血清分型及其对抗菌药物的耐药情况,为本地区预防和临床合理应用抗生素提供参考。方法收集2013年7月至2014年6月肇庆地区4家医院分离的166株社区呼吸道感染肺炎链球菌,采用荚膜肿胀试验进行血清型分析,用E试验法检测青霉素的最低抑菌浓度(MIC),用纸片法检测其他7种抗菌药物的药敏情况。结果 2 186例患儿中分离出肺炎链球菌166株,分离率为7.6%。分离株可分成6个血清型,3株菌未能分型,主要的流行血清型为19F、23F、6B、4、14和19A,7价疫苗覆盖率为94.6%。所有分离的肺炎链球菌青霉素不敏感菌株只占3.0%,对万古霉素、利奈唑胺、左旋氧氟沙星均100%敏感,对红霉素和克林霉素的耐药率高达97.6%和95.8%。结论肇庆地区肺炎链球菌临床分离株以19F、23F、6B、4、14和19A常见,7价疫苗覆盖率高,社区预防感染应选用7价疫苗。分离株对青霉素、万古霉素、利奈唑胺和左氧氟沙星敏感性高。
Objective To investigate the serotype distribution and antimicrobial resistance of S.pneumoniae isolated from children with community acquired pneumonia in Zhaoqing. Methods A total of 166 S.pneumoniae strains were collected from the children with community acquired pneumonia in 4 hospitals Zhaoqing area during July 2013 and June 2014. The isolates were serotyped by capsular swelling technique.Susceptibility to penicillin was determined by E-test; susceptibility to 7 antibiotics was determined by Kribry-Bauer method according to the standard of Clinical and Laboratory Standards Institute. Results 166 strains of S. pneumoniae were isolated from 2 186 specimens. The prevalence was 7.6%. The main serotypes of the isolates were 19 F, 23 F, 6B, 4, 14 and 19 A. There were 3 isolates belonging to non-typed. About94. 6 % of these isolates were covered by the 7-valent pneumococcal conjugate vaccine( PCV7). About 3. 0 % of all S.pneumoniae isolates were penicillin non-susceptible.No resistant strain was found to vancomycin,linezolid or levofloxacin.About 97.7% and 95.8% of the isolates were resistant to erythromycin and clindamycin. Conclusions The prevalent serotypes are 19 F,23F,6B,4,14 and 19 A in Zhaoqing,and PCV7 is an effective vaccine to prevent the S.pneumoniae infections.These S.pneumoniae strains are highly sensitive to penicillin, vancomycin,linezolid and levofloxacin.
