摘要
目的探讨富血小板纤维蛋白(PRF)在上颌窦内提升中促进骨愈合的临床作用。方法选择需行上颌窦内提升种植术患者20例,随机分为实验组12例和对照组8例,实验组上颌窦内提升时植入人工骨粉和PRF混合物并同期植入植体,对照组上颌窦内提升单纯植入人工骨粉并同期植入植体,术后1月、2月、3月通过影像学检查,观察人工骨粉降解及新骨形成情况,通过CT值来计量上颌窦内新骨密度的变化。结果 20颗种植体无脱落,植体周围创口愈合良好,无上颌窦粘膜穿孔及炎症反应。术后1月,X线示两组可见人工骨粉颗粒均未降解,新骨形成较少,两组上颌窦内新骨密度即CT值没有统计学意义。2月时,实验组人工骨粉降解较少,新骨可见形成,对照组材料仍未见明显降解,新骨形成较少,两组CT值有统计学意义。3月时,实验组人工骨粉几乎降解完全,新骨形成明显,对照组人工骨粉部分降解,有新骨形成但不明显,两组上颌窦内新骨密度即CT值有统计学意义。结论PRF在上颌窦内提升中促进新骨形成,提高新骨质量,有利于骨组织的愈合。
Objective:To discuss the clinical effects of using PRF in internal maxillary sinus floor lift to improve bone regeneration. Methods:Twenty patients participated in this study. In the experimental group,twelve patients had PRF and bone substitutes in internal maxillary sinus floor lift and simultaneous implant placement. In the control group,eight pa-tients had bone substitutes in internal maxillary sinus floor lift and simultaneous implant placement. X-ray and CBCT exam-inations were carried at 1 month,2 months,3 months after the operation to observe the condition of bone regeneration and bone substitutes degradation. The CT value was used to evaluate the density of new bone. Results:Bone regeneration healed well without any infection in all cases. At 1 month after operationm,there were no bone regeneration or bone substi-tutes degradation,and there was no statistical differences between the two CT data groups. At 2 months,there occurred a little bone regeneration and bone substitutes degradation was observed in the experimental group. No bone regeneration and no bone substitutes degradation were observed in the control group. Statistical differences were found between the two CT data groups. At 3 months,significant bone regeneration and bone substitutes degradation were deserved in the experimental group while a little but not significant were observed in the control group. Statistical differences were also found between the two CT data groups. Conclusion:PRF used in internal maxillary sinus floor lift can improve bone regeneration and osseoin-tegration.
