盐酸曲美他嗪对冠状动脉粥样硬化性心脏病心力衰竭患者疗效和对氧化应激及血液流变学的影响 被引量:27

Effect of hydrochloride trimetazidine on echocardiography, oxidative stress and hemodynamics in coronary heart disease patients with heart failure

摘要 目的 探讨盐酸曲美他嗪对冠状动脉粥样硬化性心脏病(冠心病)心力衰竭患者的疗效及其对氧化应激和血液流变学的影响.方法 选取2010年9月至2013年6月四川省崇州市人民医院诊治的82例冠心病心力衰竭患者为研究对象,随机数字表法分为观察组和对照组,各41例.对照组患者给予心力衰竭常规治疗,而观察组在此基础之上加用盐酸曲美他嗪20 mg,3次/d,疗程6个月.比较2组患者的超声心动图、血液流变学、氧化应激的改变.结果 治疗后,观察组的左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)和左心室射血分数(LVEF)改善幅度都优于对照组,差异均有统计学意义[(40 ±5)mm比(43 ±5)mm、(24±4) mm比(27 ±4)mm、(23±5)ml比(26 ±6)mm、(70±17) ml比(76±15)mm、(69±6)%比(74±6)%,均P<0.05];观察组患者全血黏度切变率、相对血液黏度切变率、血浆黏度和纤维蛋白原的改善明显高于对照组,差异有统计学意义[(3.8±1.2) mPa/s比(4.2±1.2) mPa/s、(2.3±0.5) mPa/s比(2.8±0.6) mPa/s、(1.5 ±0.5) mPa/s比(1.6±0.5)mPa/s,(3.0±1.1)g/L比(3.6±1.2)g/L,P<0.05];观察组患者氧化应激指标丙二醛、超氧化物歧化酶、一氧化氮和内皮素l的改善也明显优于对照组,差异均有统计学意义[(5.3±1.4) μmol/L比(6.3±1.3) μmol/L、(121±26) kU/L比(114±26) kU/L、(60±14) μmol/L比(68±15) μmol/L、(90±19) ng/L比(93 ±20) ng/L,均P<0.05].观察组3例出现轻微胃肠道反应(7.3%),对照组有2例出现轻微胃肠道反应(4.9%),2组间比较差异无统计学意义(x2=0.891,P=0.354).结论 盐酸曲美他嗪能够通过调节冠心病心力衰竭患者氧化应激水平,改善血流循环,进而促进心脏功能的恢复和疾病康复. Objective To explore the effect of hydrochloride trimetazidine onechocardiography,oxidative stress and hemodynamics in coronary heart disease patients with heart failure.Methods Totally 82 patients of coronary heart disease with heart failure from September 2010 to June 2013 were enrolled and randomly divided into control group (41 cases)given conventional treatment of heart failure and observation group (41 cases) given hydrochloride trimetazidine (20 mg,3 times/d) in addition to the conventional treatment.After 6 months of treatment,the changes of echocardiography,oxidative stress and hemodynamics were compared.Results After treatment,the left ventricular end diastolic diameter,left ventricular end systolic diameter,left ventricular end systolic volume,left ventricular end diastolic volume and left ventricular ejection fraction in observation group were significantly improved compared with those in control group [(40 ± 5) mm vs (43 ± 5) mm,(24 ± 4) mm vs (27 ± 4) mm,(23 ±5) mlvs (26±6)ml,(70±17) mlvs (76±15) ml,(70±17)% vs (74±6)%](P〈0.05);the whole blood viscosity,relative viscosity of blood shear rate,amplitude of plasma viscosity and fibrinogen were significantly improved [(3.8±1.2) vs (4.2±1.2),(2.3±0.5) vs (2.8±0.6),(1.5±0.5) mPa/s vs (1.6 ± 0.5) mPa/s,(3.0 ± 1.1) g/L vs (3.6 ± 1.2) g/L,P 〈 0.05].The parameters of oxidative stress,such as malondialdehyde,superoxide dismutase,nitric oxide and endothelin 1 in observation group were significantly improved than those in control group[(5.3 ± 1.4)μmol/L vs (6.3 ± 1.3)μmol/L,(114 ± 26) kU/L vs (121 ± 26) kU/L,(68 ± 15) μmol/L vs (60 ± 14) μmol/L,(90 ± 19) ng/L vs (90 ± 19) ng/L] (P 〈 0.05).Mild gastrointestinal reaction occurred in 3 cases (7.3%) of observation group and 2 cases (4.9%) of control group.Conclusion Hydrochloride trimetazidine can regulate the oxidative stress and hemodynamics to imPr
