摘要
目的 对比腹腔镜和开腹手术治疗老年急性胆囊炎的效果。方法 2010年5月-2014年3月,选取本院收治的118例急性胆囊炎并进行手术治疗的老年患者作为研究对象,按照随机分组的原则将其分为腹腔镜组和开腹组,其中腹腔镜组58例,腔镜下完成手术55例作为观察组（LC组）,开腹组60例作为对照组（OC组）。比较两组的手术时间、术中出血量、放置引流管的例数、排气时间、术后住院时间及并发症情况。结果 LC组患者的手术时间、术后排气时间、术后住院时间均短于OC组,术中失血量明显少于OC组,差异均有统计学意义（P〈0.01）,两组的放置引流管率差异无统计学意义（P〉0.05）。两组的术后总并发症发生率差异有统计学意义（P〈0.05）。结论对于老年急性胆囊炎患者,行腹腔镜胆囊切除术具有出血少,手术时间、术后排气及住院时间短的优点,值得广泛推广及应用。
Objective To compare the effect of cholecystectomy by laparoscope and laparotomy in the treatment of elderly acute cholecystitis. Methods From May 2010 to March 2014,118 elderly patients with acute cholecystitis undergone surgery were selected as research objects.They were randomly divided into laparoscope group and laparotomy group.Under laparoscope,55 out of 58 cases who were successful in surgery were classified into observation group （LC group） while 60 cases accepting open cholecystectomy were categorized into control group （OC group）.Operative time, intraoperative amount of bleeding,number of cases placing drainage tube,exhaust time, postoperative hospital stay and complications was compared between two groups. Results The operative time,exhaust time after surgery,and postoperative hospital stay in patients of LC group were all shorter than those in OC group,and the intraoperative amount of bleeding was obviously less in comparison with that in OC group,and there was a statistical difference （P〈0.01）.There was no statistical difference in number of cases of placing drainage tube （P〉0.05）.There was a statistical difference of total incidence rate of postoperative complication （P〈0.05）. Conclusion For elderly acute cholecystitis,cholecystectomy by laparoscope has advantage of little bleeding,and short operative time,postoperative exhaust time as well as hospital stay,which is worthy of expansive promotion and application.
