摘要
目的探讨血清新生血管调控相关细胞因子在渗出型年龄相关性黄斑变性(exudative age-related macular degeneration,eAMD)发病中的作用。方法采用双抗体夹心ELISA法检测51例eAMD患者(eAMD组)血清血管内皮生长因子(vascular endothelial growth factor,VEGF)、血小板衍生生长因子(platelet derived growth factor,PDGF)、基质金属蛋白酶抑制剂-1(tissue inhibitor of matrix metalloproteinase-1,TIMP-1)、内皮抑素(endostatin,ES)水平,并与20例体检健康者(对照组)进行比较。结果eAMD组血清VEGF[(165.14±100.44)ng/L]、PDGF[(62.18±16.94)μg/L]水平高于对照组[VEGF(64.90±19.74)ng/L、PDGF(39.25±8.39)μg/L](P<0.01),TIMP-1[(76.33±21.63)μg/L]与对照组[(76.61±13.25)μg/L]比较差异无统计学意义(P>0.05),ES[(139.02±40.20)ng/L]低于对照组[(160.85±43.50)ng/L],但差异无统计学意义(P>0.05);ES/VEGF(1.03±0.61)、ES/PDGF比值(2.34±0.88)均较对照组(2.78±1.25、4.34±1.69)降低(P<0.01);对照组VEGF与ES呈负相关(r=-0.700,P=0.002),eAMD组患者血清VEGF与PDGF呈正相关(r=0.610,P=0.000)。结论 eAMD患者新生血管细胞因子调节失衡与其脉络膜新生血管发生、发展相关;联合检测血清VEGF、ES、PDGF、TIMP水平对指导eAMD治疗有临床意义。
Objective To explore the clinical significance of angiogenesis regulatory cytokines in patients with the exudative age-related macular degeneration(eAMD).Methods The serum levels of vascular endothelial growth factor(VEGF),platelet derived growth factor(PDGF),tissue inhinitor of matrix metalloproteinase-1(TIMP-1)and endostatin(ES)were detected by ELISA in 51 patients with eAMD(eAMD group).Their relationships among the cytokines were analyzed and compared with 20 health volunteers(control group).Results The serum levels of VEGF and PDGF((165.14±100.44)ng/L,(62.18±16.94)μg/L)in eAMD group were significantly higher than those in control group((64.90±19.74)ng/L,(39.25±8.39)μg/L)(p〈0.01).There were no significant differences in TIMP-1((76.33±21.63)vs(76.61±13.25)μg/L)and ES((139.02±40.20)vs(160.85±43.50)ng/L)between two groups(p〈0.05).The ratios of ES/VEGF and ES/PDGF were significantly lower in eAMD group(1.03±0.61,2.34±0.88)than those in control group(2.78±1.25,4.34±1.69)(p〈0.01).VEGF was negatively correlated with ES level(r=-0.700,P=0.002)in control group,and was positively correlated with PDGF in eAMD group(r=0.610,P=0.000).Conclusion The imbalance of angiogenesis regulatory cytokines is associated with the incidence and pathogenesis of choroidal neovascularization in patients with eAMD.Combined detection of serum VEGF,ES,PDGF and TIMP-1levels is helpful to the study on the mechanism and guiding the treatment of eAMD.
