摘要
甲状腺癌术中、术后大动脉破裂较少见,发生于颈根部血管最为凶险。一旦出现大出血,应立即以手指压迫出血点,切忌盲目钳夹、填塞,并迅速输液、输血补充容量,积极抗休克治疗。破损血管多可通过直接缝合修补,行大动脉结扎风险较大,但特殊情况下仍可挽救生命。全文总结4例甲状腺癌患者术中、术后大动脉破裂出血的抢救经验及教训。
The large artery blowout during and after operation for thyroid cancer is rarely happened,and is especially fatal in lower position of neck. Once occurs,prompt press by hands instead of aimless clamps is critical,combined with anti-shock procedure like blood transfusion. In most cases,ruptured artery can be repaired directly by suture. Ligation of carotid may result in severe complication,but do effective to save lives occasionally. The experiences and lessons in rescuing bleeding induced by large artery blowout during and after operations in 4 cases with thyroid cancer were summarized in this paper.
