摘要
目的研究尿内毒素定量检测与培养结果的一致性,及对于快速诊断革兰阴性菌导致的泌尿道感染的价值。方法采用动态浊度法鲎试验定量检测240份尿液中的内毒素含量,同时进行常规尿培养和鉴定,SPSS 16.0分析检测结果,采用t检验比较不同培养结果之间内毒素检测结果差异的显著性,Kappa值评价内毒素检测和培养结果的一致性。结果培养结果为革兰阴性菌生长的尿液标本,与革兰阳性菌或真菌生长及培养2 d无菌生长的标本内毒素定量检测结果的差异有统计学意义(t=11.98,t=12.00,均为P<0.001);革兰阳性菌或真菌生长的尿液标本与培养2 d无菌生长的标本内毒素定量检测结果的差异无统计学意义(t=1.38,P=0.183)。结论尿内毒素定量检测对于快速诊断革兰阴性菌导致的泌尿道感染有着较好的应用前景。
Objective To investigate the agreement between quantitative determination of urine endotoxin and urine culture,and explore the value of urine endotoxin for rapid diagnosis of UTI( urinary tract infection) caused by Gram-negative bacteria. Methods The turbidimetric-kinetic method was used to determine urine endotoxin quantitatively. The bacteria or fungi isolated from the urine cultures were identified by VITEK 2 COMPACT system. The data were analyzed by SPSS 16. 0; the results of determination were tested by t-test. Kappa values were used to measure the agreement between quantitative determination of urine endotoxin and urine culture. Results There were significant differences in endotoxin values between the Gram-negative bacteria group and Gram-positive bacteria,fungi or None groups( t = 11. 98,t = 12. 00,P〈 0. 001). And there wasn' t significant differences between Gram-positive bacteria or fungi group and None group( t = 1. 38,P = 0. 183). Conclusion The quantitative determination of urine endotoxin is effective for rapid diagnosis of urinary tract infections caused by Gram-negative bacteria with good application prospect.
