摘要
目的:观察应用鼻内糖皮质激素治疗儿童轻度阻塞性睡眠呼吸暂停低通气综合征的疗效。方法:选取经多导睡眠监测诊断为轻度阻塞性睡眠呼吸暂停综合征85例患儿作为研究对象,以随机双盲的方法将研究对象分为两组,治疗组44例,安慰剂对照组41例,分别用糠酸莫米松鼻喷雾剂(睡前50μg每鼻孔)或安慰剂(生理盐水)治疗4周,间隔2周后,对照组给予同治疗组相同的治疗4周,治疗组停用药物治疗。每一阶段完成后均行多导睡眠监测评估和腺样体大小的影像学评估。结果:该研究中治疗组和对照组分别有36例和31例完成一阶段的治疗及评估,分别有30例与25例完成了第二阶段的治疗评估。经过4周的糠酸莫米松喷鼻治疗,儿童轻度睡眠呼吸暂停低通气综合征患者在睡眠质量(包括:睡眠潜伏期、慢波睡眠期及快眼动睡眠期)、呼吸紊乱状况(包括:呼吸暂停低通气指数、最低血氧饱和度)和腺样体大小等方面均有显著疗效,停药6周后,治疗组的睡眠质量及腺样体大小与刚停药时相比无明显变化。结论:该研究结果证实对仅有轻度阻塞性睡眠呼吸暂停的儿童给予鼻内糖皮质激素作为首选治疗具有很好的临床效果。
Objective： To observe the therapeutic effect of intranasal glucocorticoids treatment for children with mild obstructive sleep apnea hypopnea syndrome. Methods： 85 children with mild obstructive sleep apnea hypopnea syndrome diagnosed by polysomnography were recruited onto the study. All the subjects were assigned into therapy or placebo groups using double- blind, randomized meth- od. Intranasal mometasone furoate aqueous （50μg per nostril at bedtime） or placebo （0. 9% NaC1 1 spay per nostril at bedtime） were ap- plied to therapy and placebo subjects respectively for 4 weeks. The placebo group underwent the same treatment as the therapy group after 2- weeks washout period while the therapy subjects stop drug administration. Polysomnography and X-ray examination of nasopharynx were done at the end of every period. Results： At the end of the first period, 36 of the therapy subjects and 31 of the placebo subjects were successfully followed while 30 and 25 subjects were followed respectively through the whole study. The sleep quality, respiration and adenoid size of chil- dren with mild obstructive sleep apnea showed significant improvement after 4-weeks intranasal mometasone furoate aqueous administra- tion. The sleep quality, respiration and adenoid size of therapy subjects had no significant change after 6-weeks drug dismission. Conclusion： It is reasonable that intranasal glucocorticoids treatment as the first choice for children with mild obstructive sleep＇apnea hypopnea syndrome.
