摘要
目的探讨双源CT低浓度、低剂量扫描在低体质指数人群中应用的必要性与可行性。方法选取低体质指数BMI≤20患者,心率<65次/分和65≤心率<90次/分各30例,采用前瞻性心电门控扫描。低心率组采用单扇面采集;高心率组采用多扇面采集。每组再按随机性原则,15例采用370mg I/ml典比乐、120KV扫描;15例采用300 mg I/ml典比乐、100KV进行扫描。比较两组影像的质量与剂量指标,是否有统计学差异。结果两组内不同造影剂浓度对图像质量的影响:主观性评价无明显差异,图像质量均较好,客观评价图像噪声、信噪比及噪声比均无统计学差异。单扇面与多扇面相比放射剂量有明显差异CTDI及DLP分别降低20%、11%,两组数据P<0.01,有统计学差异。每组间不同KV放射剂量比较,也有明显统计学差异。100KV较120KV相比,CTDI及DLP分别降低21%、9%,两组数据P<0.01,有统计学差异。结论对于低体质指数人群,采用低浓度、低剂量的双源CT扫描程序,可以在保证成像质量的同时,有效的减低受检者的辐射剂量及造影剂的摄取,减少医源性损伤。
Objective To investigate the dual-source CT low concentrations, the necessity of low-dose CT in low body mass index and the feasibility of the application of the crowd. Methods Patients with low body mass index BMI≤20, heart rate〈65 beats/min 30 cases,65≤beats/min〈90 30 cases, prospective ECG gating. Low heart rate group using a single fan acquisition;high heart rate group with more fan acquisition. Each group then randomness principle, 15 cases using 370mgI/ml 120KV scan;15 cases using 300 mgI/ml , 100KV scan. Quality indicators were compared with the dose images, whether statistically significant. Results The effect of different concentrations of the contrast agent within the two groups on the image quality: no significant difference in the subjective evaluation of image quality are good, objective evaluation of image noise, SNR and noise ratio were not statistically different. Single and multi-fan fan has significant differences compared to the radiation dose CTDI and DLP were reduced by 20%, 11%, two sets of data P〈0.01, significant difference. Each group comparison between different KV radiation dose, there are significant statistical difference. Compared to the 100KV with120KV, CTDI and DLP were reduced by 21%, 9%, two sets of data P〈0.01, significant difference. Conclusions For the low BMI status, low concentration, low doses of dual-source CT scanner, can ensure image quality, while effectively reducing the intake of the subjects of radiation dose and contrast agent to reduce iatrogenic injury.
