摘要
目的探讨保肾消浊汤联合泼尼松对老年难治性肾病综合征的效果。方法将72例老年难治性肾病综合征患者随机分为2组,每组36例。对照组给泼尼松,观察组在对照组的基础上给予保肾消浊汤。治疗6个月,对2组24 h尿蛋白、肾功能、血脂、炎症因子指标及不良反应进行观察。结果治疗后,2组Upro、BUN、Scr水平明显降低(P<0.05),且观察组Upro、BUN、Scr水平明显低于对照组(P<0.05);治疗后,2组TC水平明显降低(P<0.05),且观察组TC水平明显低于对照组(P<0.05);治疗后,2组IL-6、TNF-α水平明显降低(P<0.05),且观察组IL-6、TNF-α水平明显低于对照组(P<0.05);观察组总缓解率为86.1%,明显高于对照组的66.7%(P<0.05);观察组不良反应率为13.9%,明显低于对照组的41.7%(P<0.05)。结论保肾消浊汤联合泼尼松治疗老年难治性肾病综合征疗效确切,可有效降低炎性反应,改善肾功能。
Objective To investigate the efficacy of Baoshen Xiaoshi decoction combined prednisone on refractory nephrotic syndrome in the elderly. Methods 72 cases of refractory nephrotic syndrome in the elderlywere randomly divided into two groups with 36 cases in each group. The patients in control group were given prednisone,while the patients in observation group were given Baoshen Xiaoshi decoction based on the treatment in the control group. After treatment six months,24 h urinary protein,renalfunction,blood lipids,inflammationfactorindicators and adversereaction of both groups were observed. Results After thetreatment,Upro,BUN,Scr levels of both groups were significantly reduced（ P 〈0. 05）,Upro,BUN and Scrlevels of observation group were significantly lower than those in the control group（ P 〈0. 05）;After thetreatment,TC levels of both groups were significantly reduced（ P 〈0. 05）,and TClevel of the observation group was significantly lower than that in thecontrol group（ P 〈0. 05）;After the treatment,IL-6 and TNF-α levels of both groups were significantly reduced（ P 〈0. 05）,IL-6 and TNF-αlevelsofthe observation group were significantly lower than those in the control group（ P 〈0. 05）;The overallrelief rate of the observation group was 86. 1%,which was significant higher than66. 7% of the control group（ P 〈0. 05）;The adversereaction rate of observation group was 13. 9%,which was significant lower than 41. 7% of the control group（ P 〈0. 05）. Conclusion Baoshen Xiaoshi decoction combined prednisone can effectively improve the symptoms of the elderpatients with refractory nephrotic syndrome,reduce inflammatoryreaction and improve the renal function.
