摘要
目的探讨多发性创伤患者血清凝血功能指标的变化规律。方法随机选取我院2013年1月至2014年1月胸腹部多发性损伤患者90例,其中开发性损伤50例,闭合性损伤40例,检测患者受伤后24 h血清凝血功能指标凝血酶原时间(PT)、部分活化凝血活酶时间(APTT)、和血小板(PLT)的变化规律,采用急性生理和慢性健康状况Ⅱ(A-PACHEⅡ)评分评估患者预后,并分析凝血功能异常程度与患者预后的关系。结果随着受伤后时间的延长,患者凝血功能指标异常程度越明显,其中出血量大于1 500 m L患者在入院后24 h凝血功能指标异常程度明显高于出血量低的患者,差异具有统计学意义(P<0.05);凝血功能重度异常组患者A-PACHEⅡ评分显著增高,方差分析显示组间比较差异显著(P<0.05)。结论多发性创伤患者出血量越高患者凝血功能异常程度越明显,随着凝血功能指标异常程度的增加,患者预后越差。
Objective To study the change rule of serum coagulation function index in patients with muhiple trauma. Methods 90 patients with multiple injuries, including 50 cases of developmental damage and 40 cases of closed injury, were randomly selected from patients in our hospital between January 2013 and January 2014. All of them were tested after 24 h injury, including the change rule of serum coagulation function indexes of prothrombin time （ PT）, activated partial thromboplastin time （APIT） and platelet （PLT）. The acute physiology and chronic health Ⅱ （ A - PACHE Ⅱ） score evaluated patients＇ prognosis, and analyzed the relationship between degree of coagulant function abnormality and the prognosis of patients. Results With the extension of time after injury, the index of blood coagulation function in patients was more obvious. When the blood loss amount was greater than 1500 ml in 24 h after injure, the abnormal degree of blood coagulation function index in these patients was significantly higher than those with less blood loss, with statistically significant difference （P 〈 0. 05 ）. There was obvious increase of A - PACHE Ⅱscore in blood coagulation function of severely abnormal patients, and the difference between groups was statistically significant （ P 〈 0.05 ）. Conclusion Patients with multiple trauma will be with more obviously abnormal coagulant function, and with the increase of abnormal degree of coagulation function indices, patients have worse prognosis.
