摘要
目的探讨整合素β1和Kindlin-2在乳腺浸润性微乳头状癌(invasive micropapillary carcinoma,IMPC)组织中的表达及意义。方法采用免疫组织化学方法检测天津医科大学肿瘤医院2005-01-01-2008-12-31收治的99例乳腺IMPC病例中整合素β1和Kindlin-2的表达,并选取该院同期诊断的132例乳腺浸润性导管癌非特殊型(invasive ductal carcinoma,not otherwise specified,IDC-NOS)病例作为对照,分析两种蛋白间及其与临床病理学特征之间的关系。结果 IMPC组织中整合素β1阳性表达率为30.3%(30/99),明显高于IDC-NOS组织的16.7%(22/132),差异有统计学意义,χ2=6.031,P=0.014;而Kindlin-2在IMPC组织中阳性表达率为64.6%(64/99),也明显高于IDC-NOS组织中的50.8%(67/132),差异也有统计学意义,χ2=4.445,P=0.035。且IMPC组织中两种蛋白的表达呈低度正相关性,r=0.212,P=0.035。IMPC组织中整合素β1的表达与HER-2(r=0.426,P<0.001)、E-CD(r=0.217,P=0.031)及淋巴结转移(r=0.207,P=0.040)呈明显的正相关,与pTNM分期无相关性(P>0.05),而Kindlin-2的表达则与HER-2(r=0.212,P=0.035)、E-CD(r=0.218,P=0.030)、淋巴结转移(r=0.294,P=0.003)以及pTNM分期(r=0.245,P=0.015)均呈正相关性;整合素β1和Kindlin-2的表达与其他临床病理学特征均无明显相关性,P>0.05。生存分析发现,IMPC组织中整合素β1阳性表达患者的无病生存期(χ2=3.982,P=0.046)和总生存期(χ2=5.407,P=0.020)均明显低于整合素β1阴性表达的患者,而Kindlin-2的表达水平与患者生存之间则无相关性,P>0.05。结论整合素β1和Kindlin-2在乳腺IMPC组织中高表达,且可能与IMPC极性倒转、集团性侵袭和高淋巴结转移密切相关,为深入探讨乳腺IMPC的发病机制和预测预后提供了新思路。
OBJECTIVE To investigate the expression and significance of integrinβ1and Kindlin-2in invasion micropapillary carcinoma(IMPC).METHODS Integrinβ1and Kindlin-2expression was analyzed in 99 IMPC cases and 132 invasive ductal carcinoma-not otherwise specified(IDC-NOS)cases via immunohistochemistry.The correlation of integrinβ1and Kindlin-2expression with clinicopathologic characteristics and between the two proteins were determined.RESULTS The rate of integrinβ1expression in IMPC(30.3%)was significantly higher than that of IDC-NOS(16.7%),the difference was statistically significant(χ^2=6.031,P=0.014).The rate of Kindlin-2expression in IMPC(64.6%)was significantly higher than that of IDC-NOS(50.8%),the difference also was statistically significant(χ^2=4.445,P=0.035).The two proteins had positive correlation in IMPC(r=0.212,P=0.035).The expression of integrinβ1in IMPC had a significant positive correlation with HER-2expression(r=0.426,P〈0.001),E-CD expression(r=0.217,P=0.031)and lymph node metastasis(r=0.207,P=0.040),but had no correlation with pTNM stages(P〉0.05).While the expression of kindlin-2in IMPC had a significant positive correlation with HER-2expression(r=0.212,P=0.035),E-CD expression(r=0.218,P=0.030),lymph node metastasis(r=0.294,P=0.003)and pTNM stages(r=0.245,P=0.015).No correlation was found between integrinβ1or Kindlin-2and other clinicopathologic characteristics(P〉0.05).Univariate analysis results indicated that the IMPC patients with integrinβ1positive expression had a lower disease-free survival(DFS)and overall survival(OS)compared to the same patients with integrinβ1negative expression(log-rank test,χ^2 were 3.982 and 5.407,P were 0.046 and 0.020,respectively),but the expression of Kindlin-2had no correlation with DFS and OS in the IMPC patients(P〉0.05).CONCLUSION This study suggests that integrinβ1and Kindlin-2were high expression in IMPC,and they may have an association with cell reversed ap
