摘要
目的探讨脑脊液肿瘤细胞整合素β_(1)活性度检测对肺癌脑膜转移诊断的应用价值。方法选取30例确诊“肺癌”并拟诊脑膜转移患者设为病例组,另选择30例脑部非肿瘤性疾病患者设为对照组。两组均通过腰穿抽取脑脊液以细胞玻片离心法收集细胞,采用免疫组化PV系列二步法、二氨基联苯胺法(DAB)显色。分析病例组脑脊液肿瘤细胞整合素β_(1)活性表达情况,比较两组脑脊液瘤细胞整合素β_(1)阳性表达情况,分析脑脊液肿瘤细胞整合素β_(1)活性高表达对肺癌脑膜转移的诊断效能。结果肺癌脑膜转移脑脊液肿瘤细胞整合素β_(1)活性主要呈高表达,在胞膜及胞浆呈现明显的棕黄色带或棕黄色颗粒。病例组整合素β_(1)活性高表达率为80.00%,显著高于对照组的3.33%,差异有统计学意义(P<0.05)。脑脊液肿瘤细胞整合素β_(1)活性高表达诊断肺癌脑膜转移的灵敏度为80.00%,特异度为96.67%,准确率为88.33%,阳性预测值为96.00%,阴性预测值为82.86%。结论肺癌脑膜转移患者脑脊液中肿瘤细胞整合素β_(1)活性度明显增高,故检测脑脊液中肿瘤细胞整合素β_(1)活性度有助于提高本病鉴别诊断能力。
Objective To discuss the practical value of detection of tumor cell integrinβ_(1)in cerebrospinal fluid for the diagnosis of meningeal metastasis of lung cancer.Methods A total of 30 cases of patients diagnosed with"lung cancer"and suspected to have meningiolar metastases were selected as the case group,and another 30 patients with non-neoplastic brain diseases during the same period were selected as the control group.In both groups,cerebrospinal fluid was extracted through lumbar puncture and cells were collected by cell slide centrifuge method.Immunohistochemical PV series two-step method and diaminobenzidine(DAB)color development method were used to analyze the activity of tumor cell integrinβ_(1)in cerebrospinal fluid.The positive expression of tumor cell integrinβ_(1)in cerebrospinal fluid between the two groups was compared,and the diagnostic efficiency of high expression of tumor cell integrinβ_(1)in cerebrospinal fluid for meningial metastasis of lung cancer was analyzed.Results The activity of tumor cell integrinβ_(1)in cerebrospinal fluid was mainly highly expressed,and the cell membrane and cytoplasm showed obvious brown-yellow bands or brown-yellow particles.The high expression rate of integrinβ_(1)activity in the case group was 80.00%,which was significantly higher than that of 3.33%in the control group,and the difference was statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value high expression of integrinβ_(1)activity in the diagnosis of meningeal metastasis of lung cancer were 80.00%,96.67%,88.33%,96.00%and 82.86%respectively.Conclusion The activity of tumor cell integrinβ_(1)in cerebrospinal fluid was significantly increased,so the detection of tumor cell integrinβ_(1)in cerebrospinal fluid is helpful to improve the differential diagnosis of this disease.
