CK5/6、CK8/18、SMA、P63、ER在乳腺导管增生性病变中的表达及意义

摘要 目的:探讨CK5/6、CK8/18、SMA、P63、ER在乳腺导管增生性病变中的表达及意义。方法:收集孝感市中心医院病理科2003-2013年存档的乳腺IDPL病理标本150例,其中UDH 21例,FEA 46例,ADH 51例,DCIS32例。采用免疫组化En Vision两步法,检测各组标本中CK5/6、CK8/18、SMA、P63、ER的表达情况。结果:UDH组CK5/6的表达明显高于ADH组、DCIS组及FEA组,比较差异有统计学意义(P<0.05);ADH组分别与DCIS组及FEA组比较,差异均有统计学意义(P<0.05);DCIS组与FEA组比较,差异无统计学意义。所有病例,基膜侧均未见CK8/18阳性表达,腔缘侧均见阳性表达,在UDH组、FEA组、ADH组、DCIS组,组间两两比较,差异均无统计学意义。可见基膜侧SMA及P63连续表达,在UDH组、ADH组、FEA组、DCIS组中,组间两两比较,差异均无统计学意义;在UDH的腔缘侧可见少许散在表达,在ADH、FEA及DCIS的腔缘侧未见表达。UDH组ER的表达分别与FEA组、ADH组、DCIS组比较,差异均有统计学意义(P<0.05);FEA组、ADH组、DCIS组,组间两两比较,差异均无统计学意义。结论:联合检测CK5/6、CK8/18、SMA、P63、ER,有助于乳腺增生性病变的病理诊断及鉴别诊断,具有一定的临床意义。 Objective:To study expression and significance of CK5/6,CK8/18,SMA,P63,ER in introductal proliferative lesions (IDPL) of the breast.Method:A total of 150 breast IDPL specimens collected from the Department of Pathology of Xiaogan Central Hospital between 2003 and 2013 were tested,including usual ductal hyperplasia (UDH) in 21, flat epithelial atypia (FEA) in 46,atypical ductal hyperplasia (ADH) in 51,and ductal carcinoma in situ (DCIS) in 32 (all low-garde).The expression of CK5/6,CK8/18,SMA,P63,ER were detected by immunohistochemical EnVision method. Result:The positive expression of CK5/6 in UDH group was significantly higher than that in ADH group,DCIS group and FEA group (P〈0.05).That in ADH group was higher than that in DCIS group and FEA group (P〈0.05).The differences were not significant between DCIS group and FEA group.The expression of CK8/18 in all cases showed no positive expression in basal and showed positive expression in luminal that in UDH group,FEA group,ADH group,DCIS group was not significantly different. The positive expression of SMA and P63 was Continuous in basal.That in UDH group,FEA group,ADH group,DCIS group was not significantly different.There were not positive expression in luminal in FEA group,ADH group and DCIS group,only showed a few scattered positive expression in UDH group (positive cell 〈5%).The expression of ER in UDH group compared with in FEA group,ADH group and DCIS group,there was significantly different (P〈0.05).That in FEA group,ADH group,DCIS group was not significantly different.Conclusion:The combined detection of CK5/6,CK8/18,SMA,P63,ER is helpful to pathological diagnosis and differential diagnosis of breast proliferative lesions.
