摘要
目的 分析不合理用药,为避免和减少错误用药提供参考。方法 依据药品说明书、《中国药典》、《抗菌药物临床应用指导原则》等相关要求,对2014年10月至12月医院静脉用药集中调配（PIVAS）药师对医嘱审核发现的不合理用药进行归纳、分析。结果 不合理用药占0.51%,主要问题为溶媒选择或用量不当、药物剂量不当、调配方法不当和配伍禁忌等。结论 经药师对医嘱审核,可以及时发现和干预不合理用药,保障临床患者用药安全。
Objective Analysis on irrational drug use, and to provide references for the avoidance and reduction of medication errors. Methods On the basis of drug instructions, ＂Chinese Pharmacopoeia＂ , ＂guiding principles of clinical application of antibacterial＂ and other relevant requirements, that occurred during 2014 from October to December in the hospital pharmacy intravenous admixture services （PIVAS） of irrational use of drugs by pharmacists on the doctor＇ s orders the audit,induction analysis .Results The irrational drug use accounted for 0.51%, the main prupose was the solvent selection or improper use, Improper dosage of druges, deployment method and Incompatibility etc.Conclusion Pharmacist advice auditing can be timely found and intervention of irrational drug use, clinical medication safety in the patient by guaranteed.
