感染性疾病患者炎性因子水平的表达与意义 被引量:19

Expression of inflammatorg cytokines in patients with infectious diseases and its significance

摘要 目的研究感染性疾病患者血液中的降钙素原(PCT)、中性粒细胞CD64(CD64)及C-反应蛋白(CRP)的表达水平,为临床感染性疾病的诊疗提供科学依据。方法随机选取医院2011年4月-2013年6月住院治疗确诊的80例感染患者和40名同时期健康体检人员,感染患者分为细菌性和病毒性感染组,每组各40例,采用罗氏定量的电化学发光法测定PCT、Becton Dikison FACS Calibur流式细胞仪测定并计算CD64百分比、芬兰QuickReader散射比浊仪测定CRP、制定工作曲线选取PCT、CD64及CRP截断值,从而分析比较细菌性、病毒性感染组和对照组的PCT、CD64及CRP表达水平和阳性率。结果细菌感染组的PCT、CD64及CRP水平的阳性率,分别为60.00%、77.50%、52.50%,均高于对照组的25.00%、30.00%、17.50%,差异有统计学意义(P<0.05);而病毒感染组CRP水平的阳性率为47.50%与对照组17.50%比较,差异有统计学意义(P<0.05),PCT和CD64水平阳性率分别为22.50%、35.00%,与对照组25.00%、30.00%比较,差异无统计学意义。结论细菌感染性疾病导致PCT、CD64和CRP表达水平升高,病毒感染性疾病导致CRP表达水平升高,PCT、CD64和CRP可用于诊断感染性疾病,其中CD64的诊断价值最高。 OBJECTIVE To analyze the expression and significance of PCT , CD64 and CRP in patients with infectious diseases to provide scientific basis for diagnosis and treatment of infectious diseases .METHODS Totally 80 patients with infectious diseases who were treated in our hospital from Apr .2011 and Jun .2013 and 40 healthy people who were examined during the same period were randomly selected .The 80 infected patients were divided into the bacterial infection group and the viral infection group with 40 patients in each group . The Roche quantitative electrochemical luminescence method was used to test PCT ,the Becton Dikison FACS Calibur flow cytometry to test CD64 and the Finland Quick‐Reader scattering turbidimeter to test CRP .The truncated value of PCT ,CD64 and CRP was determined by making a formulate work curve ,then the expression and the positive rate of PCT ,CD64 and CRP were compared among the bacterial infection group ,the viral infection group and the healthy control group .RESULTS The positive rates of PCT ,CD64 and CRP were 60 .00% ,77 .50% ,52 .50% in the bacterial infection group ,significantly higher than 25 .00% ,30 .00% ,17 .50% in the control group (P〈0 .05) . The positive rate of CRP was 47 .50% in the viral infection group and 17 .50% in the control group ,the difference was significant (P〈0 .05) ,while the positive rates of PCT and CD64 were 22 .50% ,35 .00% in the viral infection group and 25 .00% ,30 .00% in the control group .CONCLUSION Bacterial infectious diseases can lead to increased PCT ,CD64 expression and CRP levels ,viral infectious diseases can lead to increased expression level of CRP . PCT ,CD64 and CRP can be used to diagnose infectious diseases and CD64 has the highest diagnostic value .
