摘要
目的探讨小容量脑脊液置换术治疗急性自发性蛛网膜下隙出血（SAH）的临床效果。方法 54例急性自发性SAH患者随机分为2组,对照组27例采用常规治疗,观察组27例在常规治疗的基础上采用小容量脑脊液置换术治疗。观察组患者于置换术后第1~7天每日1次抽取脑脊液,分别取第1、4、7次脑脊液进行细胞学检查。对照组患者在同一时间取脑脊液。观察比较2组患者临床治疗效果、脑脊液外观评分和细胞学检查结果。结果观察组总有效率（81.48%）显著高于对照组（62.96%,P〈0.05）。第1次脑脊液细胞学检查结果显示2组患者脑脊液红细胞和白细胞数比较差异均无统计学意义（P〉0.05）,第4、7次脑脊液白细胞和红细胞数观察组显著低于对照组（P〈0.05）。2组患者第1次脑脊液外观评分比较差异无统计学意义（P〉0.05）,但第4、7次脑脊液外观评分观察组显著低于对照组（P〈0.05）。结论小容量脑脊液置换术可有效治疗急性自发性SAH。
Objective To explore the clinical treatment effect of small capacity cerebrospinal fluid replacement on a- cute spontaneous subarachnoid hemorrhage（SAH）. Methods Fifty-four cases of acute spontaneous SAH patients were ran- domly divided into observation group and control group. The control group of 27 patients were given conventional therapy. Ob- servation group of 27 cases were given small capacity of eerebrospinal fluid replacement treatment besides the conventional therapy. The patients in observation group were taken cerebrospinal fluid（CSF） once a day during the first day to the seventh day ＇after the operation, and the first, the fourth and the seventh CSF were selected for cytology detection. The patients in control group were taken CSF at the same time. The clinical treatment effect, CSF observation score and cytological examination results were compared between the two groups. Results The total effective rate of observation group（81.48% ） was significantly higher than that of control group（62.96% ,P 〈 O. 05）. The first CSF cytology result showed that there was no significant differ- ence of red blood cells and white blood cell count between the two groups, and the fourth and the seventh CSF result of red blood cells and white blood cell count in observation group were significantly lower than those in the control group（ P 〈 0. 05 ）. There was no significant difference of the CSF score of the first time between the two groups （P 〉 0. 05 ） , while the CSF score of the fourth and the seventh times in observation group was significantly lower than that in the control group（P 〈 0.05）. Conclu- sion Small volume cerebrospinal fluid replacement on acute spontaneous SAH is effective.
