摘要
目的探讨应用神经内镜技术辅助诊断及治疗脑室感染的方法及效果.方法 2008年8月至2013年9月,应用神经内镜技术对21例脑室感染患者行脑室灌洗、脑室外引流、三脑室底造瘘,并全身抗感染治疗,术后定期进行临床随访.结果脑脊液培养及涂片阳性者14例,占66.7%;脑室外引流时间为7~81 d,平均21.8 d.去除脑室外引流后行脑室腹腔分流者4例(19.0%).17例患者取得满意的疗效,死亡4例(19.0%).结论神经内镜技术可用于辅助诊断脑室感染及提高治疗效果.
Objective To discuss the effect of neuroendoscopic techniques in the accessory diagnosis and treatment of Ventriculitis. Methods From August, 2008 to September, 2013, 21 cases of ventricular infection were treated with endoscopic third ventriculostomy, endoscopic ventricular irrigation and external ventricular drainage (EVD) by rigid neuroendoscopy, in addition, all patients received systemic antibiotic therapy, postoperative follow-up was given for all patients. Results In 21 patients, 14 (66.7%) were positive in CSF bacterial culture, the EVD duration was 7 to 81d (mean 21.8 d) The secondary ventriculoperitoneal shunt was not avoided due to hydrocephalus after removal of EVD in 4 (19%) patients. 17 patients were satisfied with the treatment, 4 (19%) patients died. Conclusion Neuroendoscopic techniques can be used in accessory diagnosis of ventriculitis, and improve the effect of the treatment of severe ventricular infection.
