对经小脑延髓裂入路显微手术的临床研究

摘要 目的分析经小脑延髓裂入显微手术治疗小脑出血破入脑室的临床疗效。方法针对我院收治的小脑出血破入脑室患者77 例进行治疗观察,患者的收治时间为2015 年1 月—2018 年10 月,采用回顾性分析方式将2015 年1 月—2016 年10 月实施常规手术治疗的患者38例作为常规组,将2016 年11 月—2018 年10 月接受经小脑延髓裂入路显微手术治疗的患者39 例作为治疗组,对比两组小脑出血患者治疗后的疗效差异。结果两组治疗后的格拉斯昏迷指数评分、住院时间、术后再出血发生率、术后并发症发生率及死亡率差异有统计学意义(P< 0.05)。结论针对小脑出血破入脑室患者实施小脑延髓裂入路显微手术治疗的疗效十分明显,能明显降低患者颅内压、改善脑脊液循环及减少脑组织损伤,促进患者预后及治疗后的康复。 Objective To analyze the clinical effect of cerebellar medullary fissure in the treatment of cerebellar hemorrhage into the ventricle. Methods According to the treatment of 77 cases of small cerebral hemorrhage in our hospital, the treatment time of the patients was from January 2015 to October 2018, and 38 patients who underwent routine surgical treatment from January 2015 to October 2016 were used as the routine group. 39 of the patients treated with the microsurgical treatment of the cerebellomedullary fissure were treated as the treatment group from November 2016 to October 2018, and the treatment effect was different between the two groups. Results There were significant differences in Glass coma index score, hospitalization time, postoperative bleeding rate, postoperative complication rate and mortality between the two groups (P < 0.05). Conclusion The curative effect of cerebellar medullary microsurgery for patients with cerebellar hemorrhage and cerebral ventricle is obvious. It can significantly reduce intracranial pressure, improve cerebrospinal fluid circulation and reduce brain tissue damage, and promote prognosis and rehabilitation after treatment.
