摘要
目的：比较ITIФ6.5 mm平台种植体配对常规颈基台与配对宽颈基台用于单牙缺失修复后的边缘骨吸收量。方法：选择50例单牙缺失患者,缺失牙槽嵴解剖条件满足植入宽颈Ф6.5（种植体平台）ITI种植体手术适应症,均行非潜入式种植手术,术后3-6月制取印模,随机采用宽颈或常规颈基台各25例,常规完成金钯合金烤瓷冠修复,并随访18个月,通过X线检查骨的吸收情况。结果：全冠修复后,两组植体及修复体均未发生严重并发症,宽颈基台组在修复后3、6、12、18个月的边缘骨吸收量分别为（0.29±0.08）mm、（0.36±0.07）mm、（0.43±0.10）mm、（0.61±0.09）mm,常规颈基台组在修复后3、6、12、18个月的边缘骨吸收量分别为（0.28±0.11）mm、（0.34±0.12）mm、（0.42±0.14）mm、（0.57±0.16）mm。两组骨吸收量经t检验,差异无显著性（P〉0.05）,均数排列为：宽颈组（0.42 mm）〉常规颈组（0.40 mm）。结论：在具有垂直平台转换设计的ITIФ6.5宽颈种植体上应用常规颈基台,形成主动水平平台转换后,种植体颈周骨吸收量略有减少,但差异无统计学意义。
Objective：To compare the marginal bone loss of WN6.5 mm and RN4.8 mm abutment matching Ф6.5 mm ITI implant system to restore the loss of single-tooth. Method：50 Ф6.5 mm ITI implants were placed into 50 patients with single-tooth loss.25 cases using WN6.5 mm abutment and 25 cases using RN4.8mm abutment. They were restored by using platinum ceramic crown. All implants have been followed up,and records were kept about the marginal bone lossing through X-ray checking immediately after restoration,3,6,12 and 18 months after functional loading. The follow- up time were 2years. Result：After restored crown,all of the implants were available.No implant loss and complication occurred. The medians of marginal bone loss of Ф6.5mm 3,6,12 and 18 months after restoration was 0.29 ±0.08、0.36 ±0.07、0.43 ±0.10、0.61 ±0.09 mm,respectively. The medians of marginal bone loss of Ф4.8 mm 3,6,12 and 18 months after restoration was 0.28 ±0.11、0.34±0.12、0.42±0.14、0.57±0.16 mm,respectively. The bone loss of two group had no significant difference（P 〉0.05）.The medians of marginal bone loss of WN6.5mm abutment group was（0.42 mm） greater than RN4.8 mm abutment group（0.40 mm）. Conclusion：ITI implant system with vertical platform switching to combine initiative horizontal platform switching has no significant influence for the marginal bone loss.
