摘要
目的探讨乙肝肝硬化患者PEDF水平变化与其血小板活化、聚集的相关关系。方法依据Child-Pugh分级标准,将37例乙肝肝硬化患者分为A级15例、B级12例、C级10例,以10例正常人为对照,采用流式细胞术检测CD62P、CD63阳性百分率,血小板聚集仪测定PAg T,酶联免疫吸附法检测PEDF含量。结果 1乙肝肝硬化Child-Pugh A级、B级、C级患者血中CD62P阳性百分率显著高于正常对照组,差异有统计学意义(P<0.05);B级、C级患者CD62P的阳性表达较A级患者明显降低,差异有统计学意义(P<0.05)。乙肝肝硬化Child-Pugh A级、B级、C级患者血中血小板CD63的阳性百分率较正常对照组增高,差异有统计学意义(P<0.05)。2乙肝肝硬化Child-Pugh A级、B级、C级患者血小板PAg T明显低于对照组,差异有统计学意义(P<0.05)。3乙肝肝硬化Child-Pugh A级、B级、C级患者血清中PEDF浓度较正常对照组明显减少,差异具有统计学意义(P<0.05)。4乙肝肝硬化患者血液中PEDF的表达与CD62P负相关(r=-0.755,P<0.05),与PAg T呈正相关(r=0.791,P<0.05)。结论乙肝肝硬化患者血液中PEDF表达减少,血小板异常活化,血小板聚集功能明显下降,乙肝肝硬化患者血液中PEDF与血小板的活化和聚集有关。
Objective To explore the correlation between pigment epithelium-derived factors (PEDF) level and platelet activation, platelet aggregation in patients with hepatitis B cirrhosis. Methods According to Child-Pugh classification standard,37 hepatitis B cirrhosis patients were divided into grade A(n = 15), grade B( n = 12) , grade C( n = 10), and ten normal controls were chosen as control group. The CD62P and CD63 positive percentage were detected by flow cytometry, PAgT was measured by platelet aggregation analyzer, and the contents of PEDF was detected by enzyme-linked immunosorbent method. Results ①The CD62P positive percent- ages in hepatitis B cirrhosis patients were significantly higher than in normal controls (P 〈 0.05). Compared with grade A patients, the CD62P positive percentages in grade B and C patients were decreased obviously( P 〈0.05 ). The CD63 positive percentages in hepati- tis B cirrhosis patients were higer than that in normal controls( P 〈 0.05 ). ②The PAgT in hepatitis B cirrhosis patients was significant- ly lower than that in normal controls(P 〈 0.05). ③The plasma concentration of PEDF in hepatitis B cirrhosis patients was decreased significantly compared with normal controls(P 〈 0.05). ④In hepatitis B cirrhosis patients, PEDF expression was negatively correlated with CD62P (r= -0.755,P〈0.05), and positively correlated with PAgT(r =0.791,P 〈0.05). Conclusion In hepatitis B cir- rhosis patients, the PEDF expression reduces, the platelets are abnormally activated, and the platelet aggregation function is droped. The plasma levels of PEDF in hepatitis B cirrhosis patients could influence the platelet activation and platelet aggregation, which may partly explain why the bleeding risk increases in hepatitis B cirrhosis patients.
