摘要
目的探讨腹腔镜胆囊切除辅助小切口胆总管探查治疗胆囊结石合并胆总管结石的临床效果。方法选择我院肝胆外科2012年2月至2014年6月期间所收治98例胆囊结石合并胆总管结石患者作为研究对象,分为两组各49例。对照组行开腹手术,观察组行腹腔镜胆囊切除辅助小切口胆总管探查术,比较两组术中出血量、术后排气时间、导尿管留置时间、住院时间及并发症发生率。结果观察组术中出血量、术后排气时间、导尿管留置时间、住院时间分别为(44.6±9.5)m L、(1.4±0.3)天、(2.3±0.4)天、(4.7±1.1)天,均显著低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为4.1%,显著低于对照组16.3%,差异有统计学意义(P<0.05)。结论胆囊结石合并胆总管结石患者应用腹腔镜胆囊切除辅助小切口胆总管探查进行治疗的手术创伤更轻,患者术后恢复更快,住院时间得以缩短,并发症发生率更低,临床借鉴价值高。
Objective To explore the clinical effect of laparoscopic cholecystectomy assisted with mini-lap exploration of common bile duct for the treatment of cholecystolithiasis and choledocholith. Methods Ninety-eight cases of cholecystolithiasis and choledocholith were divided into the control group （n = 49, open surgery） and the observation group （n = 49, laparoscopic cholecystectomy assisted with mini-lap exploration of common bile duct）. The intraoperative blood loss, postoperative exhaust time, in-dwelling urethral catheter time, length of hospital stay and incidence of complications were compared between two groups. Results The intraoperative blood loss, postoperative exhaust time, in-dwelling urethral catheter time, length of hospital stay of observation group were （44.6 ± 9.5） mL, （1.4 ± 0.3） days, （2.3 ± 0.4） days and （4.7 ± 1.1） days respectively, significantly lower than those of control group （all P 〈0.05）. The incidence of complications of observation group was 4.1%, significantly lower than 16.3% of control group （P 〈0.05）. Conclusions Laparoscopic cholecystectomy assisted with mini-lap exploration of common bile duct for the treatment of cholecystolithiasis and choledocholith has less invasive, faster recovery, shorter length of hospital stay, and lower incidence of complications.
