摘要
目的研究老年慢性阻塞性肺疾病(COPD)肠内营养支持对机体免疫力、营养指数及预后的影响。方法选择重庆永荣矿业有限公司总医院广顺分院2012年6月至2013年6月住院的COPD患者112例作为研究对象,采用随机数字表法分为两组:研究组(62例)采用早期肠内鼻饲营养支持,对照组(50例)采用肠外营养支持治疗。治疗前后对患者营养指标、免疫指标进行检测,同时观察两组患者平均住院时间、机械通气时间等指标。结果治疗后,两组患者血红蛋白、转铁蛋白、淋巴细胞数、血浆总蛋白值均较治疗前升高(P<0.05),但组间比较差异无统计学意义(P>0.05);Ig A、Ig G、CD3+、CD4+均高于治疗前,Ig M低于治疗前,但组间比较差异无统计学意义(P>0.05);研究组患者的机械通气时间、平均住院时间均显著短于对照组,动脉血氧分压显著高于对照组,动脉血二氧化碳分压显著低于对照组,差异均有统计学意义(均P<0.05)。结论肠内营养支持有利于老年COPD患者营养状况的改善,从而提高其免疫功能,缩短机械通气时间和住院时间。
Objective To study the impact of enteral nutrition support on the immunity,nutrition index and prognosis in elderly patients with chronic obstructive pulmonary disease （COPD）. Methods A total of 112 COPD patients hospitalized in General Hospital of Chongqing Wing Mining Co. Ltd. Guangshun Branch from Jun. 2012 to Jun. 2013 were selected as the research objects, and divided into two groups by random number table method：patients in the study group （62 cases） were given early nasogastric enteral nutrition support,while patients in the control greup（50 cases） were given parenteral nutrition support. The nutrition indexes, immune parameters of the two groups before and after treatment were detected, while the average length of hospital stay, duration of mechanical ventilation and other indicators of the two groups were observed. Results After treatment, levels of hemoglobin, transferrin,lymphocyte count, plasma total protein of the two groups were all increased compared with those before treatment（ P 〈 0. 05 ）, but there were no sta- tistically significant differences between two groups （ P 〉 0. 05 ） ; levels of IgA, IgG, CD3 ＋ , CD4 ＋ of the two groups were higher than those before treatment,!gM was lower than that before treatment,but there were no statistically significant differences between the two groups （P 〉 0. 05 ） ;the mechanical ventilation time, aver- age hospitalization time of the study group was significantly less than those of the control arterial partial pres- sure of oxygen level of the study group was significantly higher than that of the control group, arterial partial pressure of carbon dioxide level was significantly lower than that of the control group, the differences were sta- tistically significant （ P 〈 0. 05 ）. Conclusion Enteral nutritional support will help to improve the nutritional status of elderly COPD patients to enhance their immune function, shorten the machine time and hospital stay.
