摘要
目的通过检测糖尿病患者和健康对照者尿液中砷、镉、钴、铬、铜、铁、锂、镍、锰、铅、硒、钒、锌13种微量元素的含量,统计分析两组人群尿液元素的差异性,以及元素与血糖的相关性。方法随机选取了100例糖尿病患者,选取性别和年龄相匹配的健康对照者110人,收集研究对象的晨尿,将尿液用3%(W/V)的硝酸及内标液等稀释20倍后直接用电感耦合等离子体质谱仪进行检测。用SAS 9.2对数据进行统计学分析。结果糖尿病患者尿液中砷、铜、镍、锰、硒、钒、锌的含量比健康对照组高,差异具有统计学意义(P<0.05)。其中锌在糖尿病组和健康对照组的中位数和四分位数M(Q25,Q75)分别是481.98(315.27,674.79)μg/g肌酐、323.43(200.16,418.60)μg/g肌酐,差异具有统计学意义(Z=5.74,P<0.0001)。Spearman等级相关分析显示上述7种元素与空腹血糖呈正相关,其中锌与血糖的相关性最强(rs=0.37,P<0.0001)。结论砷、铜、镍、锰、硒、钒、锌在糖尿病患者与健康者尿液中的含量存在统计学差异,且与空腹血糖呈正相关。
Objective To investigate on the relation between urinary element contents and plasma glucose, 13 traceelements(arsenic, cadmium, cobalt, chromium, copper, iron, lithium, nickel, manganese, lead, selenium, vanadium and zinc) inurine of diabetic patients and healthy control subjects have been determined and compared between groups statistically.Methods Morning urine samples from 100 diabetic patient and 110 health controls matching with age and sex were collected.Inductively coupled plasma mass spectrometry(ICP- MS) has been applied to detect the concentrations of trace elementsdirectly after diluting 20-fold with 3%(W/V) nitric acid. Statistical analysis was performed by SAS 9.2. Results The As, Cu,Ni, Mn, Se, V and Zn concentrations in diabetic patients were high than healthy control group with significant difference(P〈0.05). Especially, the median(25%, 75%) of zinc in diabetes patients and healthy control group were 481.98(315.27,674.79),323.43(200.16,418.60)μg/g creatinine with statistically difference(Z=5.74, P〈0.0001). The urinary contents of As, Cu, Ni,Mn, Se, V and Zn showed positive relationship with fasting plasma glucose by spearman rank correlation analysis, among whichZn showed the strongest correlation(rs=0.37,P〈0.0001). Conclusion There were significant differences on the urinary As,Cu, Ni, Mn, Se, V and Zn concentrations between diabetic patients and health controls, which showed positive correlation withfasting blood glucose.
