摘要
目的探讨老年女性血尿酸(SUA)与代谢综合征(MS)及各代谢组分的相互关系。方法将147例年龄大于60岁的女性MS患者根据合并代谢组分的数量分为MSI、II、11I组,比较3组之间的SUA差异;采用四分位法将SUA分为4组,比较4组间代谢组分的差异。采用Logistic回归分析MS各代谢组分与SUA之间的相互关系。结果MSI、Ⅱ组与MS1N组的SUA水平比较,差异有统计学意义(P〈0.05);SUA4个区间q25(〈44.31mg/L)、q50(44.31~53.24mg/L)、q75(〉53.24~62.68mg/L)和q100(〉62.68mg/L)患者的腰围、收缩压(SBP)、舒张压(DBP)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL—C)和空腹血糖(FPG)比较,差异均有统计学意义(P〈0.05)。与SUA〈44.31mg/L比较,SUA44.31~53.24、〉53.24~62.68、〉62.68mg/L3个区间的MS发生风险分别为1.87(95%CI1.35~2.59)、2.71(95%CI1.88~3.91)和4.88(95%Cj3.12~7.63)。suA的危险因素有腰围(OR=2.56,95%CI1.52~4.31)、TG(OR一2.21,95%CI1.63~3.00)、SBP(OR=1.68,95%cI1.33~2.12)、DBP(OR=1.84,95%CI1.41~2.40)、FPG(OR=2.16,95%CI1.68~2.78)和代谢组分合并数量(OR=2.87,95%CI1.89~4.36),SUA的保护因素有HDL—c(OR=0.48,95%C10.29~0.79)。结论sUA和MS各组分可能存在着相互作用。
Objective To discuss the relationship between serum uric acid(SUA) and the metabolic components in metabolic syndrome(MS). Methods 147 female cases of MS aged 〉60 year old were divided into MS I group, MSⅡ group and MSⅢ group according to numbers of metabolic components, and the levels of SUA were compared among the three groups. These patients were also subdivided into 4 groups according to the SUA levels by the quartile division,and the differences of the metabolic compo- nents were compared among the 4 groups. The relationship between SUA and various metabolic components of MS was analyzed by Logistic regression analysis. Results The SUA level had statistical difference among the MS I group, MS II group and MS Ⅲ group(P〈0.05) ;the four SUA intervals of q25 (〈 44.31 mg/L), q50 (44. 31 - 53. 24 mg/L), q75 ( 〉 53. 24 - 62. 68 mg/L) and q100(〉62.68 μg/L) showed significant differences in waistline,SBP,DBP,TG, HDL-C and FPG(P〈0.05). Compared with the SUA interval of 〈44.31 mg/L,the occurrence risk of MS in the SUA intervals of(44.31-53.24) ,〉(53. 24-62. 68) and 〉62.68 mg/L were 1.87(95%CI1.35-2.59) ,2.71(95%CI 1.88-3.91) and 4.88(95%CI 3.12-7.63) ,respectively. Waistline(OR= 2.56,95%CI 1.52-4.31),TG(OR=2.21,95%CI 1.63-3.00),SBP(OR=I. 68,95%CI 1.33-2.12),DBP(OR=l. 84,95%CI 1.41-2.40) ,FPG(OR=2.16,95%CI 1.68-2.78) and numbers of metabolic components(OR= 2.87,95%CI 1.89-4.36) were the risk factors for SUA,and the HDL-C was the protective factor(OR=0.48,95%CI 0. 29-0. 79). Conclusion There may be in- teraction between SUA and the metabolic components of MS.
