摘要
[目的]探讨临床护理路径在2型糖尿病病人规范化管理中的效果及临床意义。[方法]随机数字表法将2011年9月—2013年9月住院治疗的2型糖尿病病人200例分为研究组和对照组各100例,对照组实行常规护理,研究组实行临床护理路径,比较两组病人的住院成本、满意度、自我管理行为评分和血糖血脂控制情况。[结果]研究组平均住院日、住院费和药占比均明显低于对照组,病人及家属满意度高于对照组,差异有统计学意义(P<0.05)。实行临床护理路径1个月后,研究组病人糖尿病自我管理行为评分均高于对照组,差异有统计学意义(P<0.05);空腹血糖(FBG)、餐后血糖(PBG)、糖化血红蛋白(HbA1c)、胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)水平低于对照组,差异有统计学意义(P<0.05);两组体重指数(BMI)水平比较差异无统计学意义(P>0.05)。[结论]临床护理路径规范化管理2型糖尿病病人,能够明显提高护理质量,改善护患关系,增强病人糖尿病自我管理水平,更好地控制血糖和预防并发症,有助于降低医疗成本,节约医疗资源。
Objective：To probe into the effect and clinical significance of clinical nursing pathway in standardized management of patients with type 2 diabetes.Methods：A total of 200 cases of hospitalized patients with type 2 diabetes were divided into research group and control group with the random number table method from Sep-tember in 201 1 to September in 2013,100 cases in each.The patients in control group were given routine nurs-ing,and the clinical nursing pathway was carried out for the patients in research group,then to compare the hos-pitalization cost,satisfaction,self management behavior score and glucose and lipid control of patients between both groups.Results：In research group,the average length of stay,hospital costs and the proportion of drugs were significantly lower than that in control group,and the satisfaction of patients and their family members was higher and the difference was statistically significant （P 〈0.05）.After the implementation of clinical nurs-ing pathway for a month,the diabetes self management behavior score of patients in research group was higher than that in control group and the difference was statistically significant （P 〈 0.05 ）.Fasting blood glucose （FBG）,postprandial blood glucose （PBG）,glycosylated hemoglobin （HbA1c）,cholesterol （TC）,three glycerol （TG）,low density lipoprotein （LDL）levels were lower than those in control group,and the difference was sta-tistically significant （P 〈0.05）.There was no statistically significant difference in Body mass index （BMI）be-tween both groups （P〉0.05）.Conclusion：The standardized management of patients with type 2 diabetes by clini-cal nursing pathway can significantly enhance the quality of nursing,improve the relationship between nurses and patients,enhance the level of self management in patients with diabetes mellitus,better control and pre-vent the blood glucose and complications.It also help to reduce medical costs and save the medical resources.
