摘要
目的探讨Ⅰ型糖尿病患儿血脂异常的特征及其血糖与血脂之间的相互关系。方法选取2010年9月至2013年9月入住我院的46例Ⅰ型糖尿病患儿为研究对象。根据是否合并有酮症酸中毒(DKA),将患儿分为DKA组(18例)和非DKA组(28例),比较两组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、空腹血糖(FBG)以及糖化血红蛋白(Hb A1c)水平差异。再根据血脂水平将患儿分为血脂正常组(17例)和血脂异常组(29例),比较两组FBG及Hb A1c水平差异。结果 DKA组患儿TG、LDL以及FBG水平均显著高于非DKA组,差异具有统计学意义(P<0.05);但两组TC、HDL及Hb A1c水平相比,差异无统计学意义(P>0.05)。血脂异常组有15例(51.7%)合并DKA,显著高于血脂正常组的3例(17.6%),差异具有统计学意义(P<0.05);但两组FBG和Hb A1c水平相比并无显著差异(P>0.05)。结论血脂异常的Ⅰ型糖尿病患儿更易发生酮症酸中毒。
Objective To explore the characteristics of dyslipidemia and the relationships between glucose and lipid in children with typeⅠdiabetes mellitus. Methods 46 cases of children with typeⅠdiabetes mellitus admitted to our hospital from September 2010 to September 2013 were selected as research objects and divided into DKA group (18 cases) and non-DKA group (28 cases) according to whether complicating diabetic ketoacidosis (DKA). The level of total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), high density lipoprotein (HDL), fasting blood glucose (FBG) and hemoglobin A1c (HbA1c) were compared. Patients were also divided into ortholiposis group (17 cases) and dyslipidemia group (29 cases) according to the lipid level, the level of FBG and HbA1c between two groups were compared. Results The level of TG, LDL and FBG in DKA group were significantly higher than those of non-DKA group, with statistical difference (P〈0.05);but the level of TC, HDL and HbA1c between two groups had no statistical difference (P 〉0.05). In dyslipidemia group, 15 cases complicating DKA with the rate being 51.7%, higher than 3 cases (17.6%) in ortholiposis group, the difference was statistical (P〈0.05);the level of FBG and HbA1c in two groups had no statistical difference (P〉0.05). Conclusions Children with typeⅠdiabetes mellitus complicated with dyslipidemia are more susceptible to diabetic ketoacidosis.
