摘要
目的探讨重症监护室(ICU)内肺内感染发生情况及防治措施。方法分析2013年10月-2014年10月苏州大学附属第二医院ICU收治的168例患者临床资料,依据是否存在肺内感染,分为肺内感染组(18例)和非感染组(150例)。观察两组患者的气管切开、机械通气、侵入性操作、全胃肠外营养及应用激素情况;采用Logistic回归分析ICU患者肺内感染高危因素情况。结果肺内感染组气管切开、机械通气、侵入性操作、全胃肠外营养比例明显高于非感染组,差异均有统计学意义(P〈0.05)。两组患者应用激素比例差异无统计学意义(P〉0.05)。Logistic回归分析结果 ,气管切开、机械通气、侵入性操作、全胃肠外营养是ICU患者肺内感染的高危因素(OR=6.158、4.187、6.078、5.876,95%CI=1.987-31.815、1.854-22.648、1.223-30.210、1.852-26.138,P=0.037、0.021、0.030、0.026、0.061)。结论气管切开、机械通气、侵入性操作、全胃肠外营养是ICU患者肺内感染的高危因素,给予针对性的防治措施,可以降低ICU患者肺内感染的发生率。
Objective To approach relevant factor and controlling measure of lungs infection occurrence in ICU.Methods From October 2013 to October 2014, in the Second Affiliated Hospital of Soochow University, clinical data of168 patients from ICU were selected and divided into two groups, according to lungs infection or not, lungs infection group were 18 cases and no infection group were 150 cases. The tracheotomy, mechanical ventilation, invasive operation, total parenteral nutrition and hormone application of two groups were detected, and lungs infection risk factors were analyzed Logistic regression. Results The rates of tracheotomy, mechanical ventilation, invasive operation, total parenteral nutrition proportion in lungs infection group were higher than those in no infection group, the differences were statistically significant(P〈0.05). The application of hormone proportion of two groups was not statistically significant different(P〉0.05). The Logistic regression analysis result showed, tracheotomy, mechanical ventilation, invasive operation, total parenteral nutrition were lungs infection risk factors(OR=6.158, 4.187, 6.078, 5.876; 95%CI = 1.987-31.815, 1.854-22.648, 1.223-30.210, 1.852-26.138, P = 0.037, 0.021, 0.030, 0.026, 0.061). Conclusion The tracheotomy, mechanical ventilation, invasive operation, total parenteral nutrition are lungs infection risk factors, which should be given corresponding prevention measures, so as to decrease the incidence of lung infection of ICU patients.
