摘要
目的:了解贵州省基层医院护理人员锐器伤防护行为,为职业防护管理提供依据。方法:采用自行设计的"医务人员职业暴露及防护情况调查表",对贵州省第十期重症医学护士专科培训班的31家医院58例护理人员进行调查,使用Spss软件对结果进行分析。结果:基层医院护理人员使用锐器盒依从性较低,治疗车上备锐器盒(3.37±1.76)分,95%CI(2.88,3.86);治疗盘内备锐器盒(2.26±1.56)分,95%CI(1.83,2.69);为病人动静脉抽血、注射及拔针时使用带锐器盒的治疗车或治疗盘均低于3.0分,锐器盒满3/4更换(3.98±1.29)分,95%CI(3.63,4.33);接触锐器时的防护行为不规范,使用后的注射针不回套针帽(3.19±1.52)分,95%CI(2.78,3.6);需要套针帽的情况下用单手套针帽(3.36±1.24)分,95%CI(13.01,3.71);使用后注射器针头在原地立刻放入锐器盒(3.31±1.55)分,95%CI(2.88,3.74);用锐器盒卡槽分离注射器针头(3.3±1.4)分,95%CI(2.91,3.69);拔头皮针后不将针头插入瓶口或莫非氏滴管内(3.65±1.45)分,95%CI(3.24,4.06);折安瓿时用纱布包裹(2.4±1.27)分,95%CI(2.05,2.75)。结论:基层医院护理人员锐器伤防护行为较差,职业防护管理有待加强。
Objective： To investigate the protective behavior with sharp instrument injuries of among nursing staffs in Guizhou primary hospital,which provides the reference for taking effective measures to reduce incidences of the sharp injuries. Methods： ＂The medical staff occupational exposure and protection questionnaire＂was designed to investigate the nurse specialists in the fifth Intensive Care Training,58 nurses from 31 primary hospitals.Spss software was used to analyze the results. Results： The compliance to use sharp utensil collector among nurses was low. The score of using sharp utensil collector on the treatment cart was（ 3. 37 ± 1. 76）,95% CI（ 1. 83,2. 69）. The score of using sharp utensil collector on the treatment plate was（ 2. 26 ± 1. 56）,95% CI（ 1. 83,2. 69）. The score of using sharp utensil collector on the treatment cart or plate was lower than 3 points when blood drawing,injection and pulling needles. The score of changing the sharp utensil collector was（ 3. 98 ± 1. 29）,95%CI（ 3. 63,4. 33） when it was 3 /4 full. Nurses＇ protective behavior was not standard. The score of not putting needle cap back was（ 3. 19 ± 1. 52）,95% CI（ 3. 63,4. 33）. The score of putting needle cap back with single hand when it necessary was（ 3. 36 ± 1. 24）,95% CI（ 3. 01,3. 71）. The score of putting needles immediately into sharp utensil collector after using was（ 3. 31 ± 1. 55）,95% CI（ 2. 88,3. 74）. Using the sharp utensil collector to separate needle was 3. 3 ± 1. 4,95% CI（ 2. 91,3. 69）. The score of not inserting needle into Murphy＇s dropper bottle was（ 3. 65 ± 1. 45）,95% CI（ 3. 24,4. 06）. The score of breaking off ampoules wrapped with gauze was（ 2. 4 ±1. 27）,95% CI（ 2. 05,2. 75）. Conclusions： The protective behavior among nurses on sharp instrument injuries is poor in primary hospitals. The occupational protection management needs to be strengthened.
