摘要
目的探讨"浙江省血液应急联动保障机制"的可行性和可靠性。方法对2011年温州"7·23"动车事故(简称"7·23"事件)的血液库存、血液联动保障等数据进行回顾性分析,并将血液出库、血液运输等数据与2012年7月浙江省血液应急联动演练数据进行统计分析。结果 "7·23"事件22∶00事发地血站红细胞成品库378 U,应急血液联动保障红细胞890 U、血浆1 278 U,事发后红细胞供应峰值出现于d1和d4,分别为595 U和698 U;事发前后15 d红细胞日平均发血量(U)分别为201±35和306.6±165.6,(P<0.05),血浆和血小板日平均发用量在事故前后无明显;"7·23"事件和应急演练的血液应急出库效率(袋/min)分别为0.38±0.13和3.45±3.04,血液运输效率(min/km)分别为0.85±0.12和0.79±0.16。结论 "浙江省血液应急联动保障机制"具有很强实践操作性和示范性,也为国家层面血液应急联动机制的建立做了前期探索。
Objective To explore the feasibility and reliability of ＂Zhejiang Quick Response Mechanism for Emergency Blood Distribution＂. Methods A retrospective analysis was conducted on the blood inventory and regional emergency blood distribution after Wenzhou Railway Crash on July 23 rd,2011. Further analysis was conducted on the volume of outbounding blood and the efficiency of transportation between ＂7. 23 ＂and the emergency blood distribution drill in 2012. Results At 22∶00 on July 23 rd,2011,RBC inventory in Wenzhou Station had378 U. The total quantity of RBC and Plasma for emergency blood distribution were 890 U and 1 078 U,respectively. RBC supply had two peaks on the first day and on the fourth day after the crash,at 595 U and 698 U respectively. Fifteen days before the accident,the average daily RBC supply at Wenzhou station was（ 201 ± 35） U. After the accident,the rate went steadily up to（ 306. 6 ± 165. 6） U（ P〈0. 05）. The demand of PLT and plasma had no significant difference. In railway crash and emergency drill,emergency outbound blood efficiency were（ 0. 38 ± 0. 13） bags / min and（ 3. 45 ± 3. 04） bags /min,respectively. The blood transport efficiency were（ 0. 85 ± 0. 12） min / km and（ 0. 79 ± 0. 16） min / km,respectively.Conclusion ＂Zhejiang Quick Response Mechanism of Emergency Blood Distribution＂has a strong and practical operability and guidance and could lend insights into the preliminary exploration on national blood emergency distribution mechanism.
