摘要
目的探讨凋亡抑制蛋白Survivin在胃癌中的表达及与预后的关系,为指导胃癌的诊断、临床治疗及判断预后提供理论依据。方法采用免疫组织化学方法分别检测60例胃癌、60例癌旁组织及20例正常胃黏膜组织中Survivin的表达,结合临床病理特征和随访资料,建立Cox模型进行分析。结果 Survivin在胃癌组织、癌旁组织及正常胃黏膜组织中的表达率分别为55.0%、43.3%和0.0%,差异有统计学意义(P<0.05)。Survivin在胃癌组织中的表达与临床分期、组织学分级、浸润深度、淋巴结转移及脏器转移呈正相关(P<0.05);Survivin的表达与性别、年龄、肿瘤直径及Her-2状况不相关(P>0.05);Survivin低表达组患者1、3和5年的生存率显著高于Survivin高表达组患者,差异有统计学意义(P<0.05)。多因素Cox回归分析显示,脏器转移、淋巴结转移、Survivin表达、肿瘤直径及临床分期是影响患者生存率的独立危险因素。结论 Survivin的异常高表达与胃癌的发生、发展及预后密切相关,提示Survivin的表达可以作为判断胃癌进展及预后的指标之一。
[ Objective ] To investigate the expression of the apoptosis inhibitor Survivin in gastric cancer and its correlation with prognosis so as to provide a theoretical basis for gastric cancer diagnosis, clinical treatment and prognosis. [ Methods ] The expression of Survivin was detected by immunohistochemistry Strepta- vidin-peroxidase method in adjacent-carcinoma tissues of 60 eases, normal mucosa of 20 cases and gastric cancer of 60 cases. Together with clinical and pathological features and follow-up data, the Cox model analysis was established. [Results] The positive expression of Survivin was 55.0% （33/60） in the tissues from gastric cancer, 43.3% （26/60） in the corresponding adjacent gastric cancer tissues, and 0.0% （0/20） in the nor- mal tissues with statistical differences （P〈 0.05）. The expression of Survivin in the gastric cancer was signifi- cantly related with clinical stage, histological grade, depth of invasion, auxiliary lymph node metastasis and organ metastasis （P 〈 0.05）; while the expression of Survivn did not show correlation to sex, age, diameter of tumors, and Her-2 status （P 〉 0.05）. The 1, 3, 5-year survival rates of the patients with low expression of Survivin were significantly higher than those of the patients with high expression with significant differences （P〈 0.05）. Multivariate Cox regression analysis showed that organ metastasis status, lymph node metastasis, Survivin expression,diameter of tumor and clinical stage were the independent risk factors affecting the survival rate of the patients. [Conclusion] The abnormal expression of Survivin is closely related to the development and prognosis of gastric cancer, which suggests that Survivin expression could be used as an indicator for determination of the progress and prognosis of gastric cancer.
