摘要
目的观察Er∶YAG激光联合生物活性玻璃对牙本质小管的封闭效果。方法收集人离体前磨牙56颗,制备56个牙本质盘并用0.1%EDTA脱矿形成敏感牙本质模型。将其随机分成4个大组:A组、B组、C组和D组,每组再随机分2个亚组:A1组、A2组;B1组、B2组;C1组、C2组;D1组,D2组。共计8组,每组7颗牙。其中A2~D2组采用抛光轮创造出玷污层。A组:单纯采用生物活性玻璃糊剂涂抹,作用时间5 min。B组:单独采用Er∶YAG激光照射,照射时间60 s,能量80 mJ,频率2 Hz,功率0.15 W。C组:采用Er∶YAG激光联合生物活性玻璃,生物活性玻璃涂抹时间5 min,冲洗30 s后,行Er∶YAG激光,照射时间60 s,能量80 mJ,频率2 Hz,功率0.15 W。D组:为空白对照,不做任何处理。结果 SEM观察显示,牙本质小管的暴露面积C10.05),但相比于与A2组、D2组均有显著性差异(P〈0.05)。玷污层组(2组)的牙本质小管封闭效果均显著优于相同处理下的无玷污层组(1组),(P〈0.05)。结论 Er∶YAG激光单独使用或与生物活性玻璃联合应用均能取得较好的牙本质小管封闭效果,玷污层的存在能够加强这种效果。这为牙本质过敏症的治疗提供了新思路,但临床疗效有待进一步研究。
Objective The aim of this in vitro study was to evaluate occluding effects of Er ∶ YAG laser and bioactive glass dentifrice application separately and in combination on the dentinal tubules.Methods Fifty six human premolars were collected in accordance with the inclusion criteria.All of the dentin discs were prepared and demineralized by 0.1% EDTA.The specimens were randomly divided into 4 groups(A-D),Group A: bioactive glass paste treatment for 5 minutes; Group B: Er ∶ YAG laser irradiation for 60 seconds(80 m J,2 Hz and 0.15 W); Group C: bioactive glass followed by Er ∶ YAG laser; Group D(control group) : no treatment.Then each group was randomly divided into 2 subgroups(A1-D1,A2-D2).Smear layers were created in subgroup A2-D2 by using the polisher.Results Compared with bioactive glass dentifrice application alone,all other treatments delivered significantly more occluded dentin orifices(P〈0.05).In terms of the occlusion effect,Group B1 was significantly higher than Group C1(P〈0.05),but there was no significant difference between Group B2 and C2(P〈0.05).Groups with a smear layer yielded significantly more occluded dentinal orifices than other groups when received the same treatment.Conclusions Laser irradiation alone or combined with bioactive glass dentifrice is proved to be superior to bioactive glass dentifrice alone on dentinal tubuli orifice occlusion.They have the potential to improve the outcome of treatment for dentin hypersensitivity.However,further study is needed to present the long-term effects.
