良性脑膜瘤磁共振表现及病理和肿瘤复发的相关性分析 被引量:6

摘要 目的研究良性脑膜瘤磁共振表现及病理病程和肿瘤复发的关系。方法选取2011年1月~2014年1月期间来进行治疗良性脑膜瘤的患者60例,其中30例患者为原发性脑膜瘤为对照组,其余30例患者为复发性脑膜瘤为观察组。将两组患者的性别、年龄、肿瘤形态等指标作为自变量,进行多元Logistic回归分析。并对两组患者的病理类型、累及部位及MRI表现情况进行对比和分析。结果 Logistic回归分析结果显示,当累及颅内外沟通瘤或颅内重要结构时良性脑膜瘤患者复发风险高。观察组患者有颅内外沟通瘤和颅内重要结构受累的几率显著高于对照组患者,观察组患者肿瘤位于窦旁和颅底的患者显著多于对照组患者,比较两组间差异具有统计学意义(P〈0.05)。两组患者脑膜瘤所在位置、病理类型、肿瘤形态、瘤周水肿程度和体积大小均无统计学意义(P〉0.05)。结论磁共振能在预测良性脑膜瘤的预后和复发中发挥重要作用。 Objective To study relationship among the MRI manifestations,pathology and relapse of benign meningiomas.Methods 60 patients with benign meningioma patients treated during aperiod from January 2011 to January 2014 were selected,in which 30 patients with primary meningioma were taken as the control group,and the remaining 30 patients with recurrent meningioma as the observation group.The parameters,in terms of gender,age,tumor morphology and other indicators of patients in two groups,taken as independent variables,were subjected to multivariate Logistic regression analysis.Also,pathological types,the involved locations,and MRI findings of patients in two groups were compared and analyzed.Results Logistic regression analysis showed that,when the aneurysms involved the intracranial and extracranial communication or important intracranial structurs,the benign meningiomas had high risk of recurrence.The rate of intracranial and extracranial communicating tumors or the involvement of important brain structure of patients in observation group was significantly higher tha that of patients in control group.The patients with tumors located beside the sinus and in skull base in the observation group were more than those in the control group of patients,with statistically differences(P〈0.05).However,the location,histological type,tumor morphology,degree of peritumoral edema,and volume size of patients with meningioma in two groups were not shown statistically different(P〉0.05).ConclusionMRI can predict prognosis and recurrence of benign meningiomas,thereby playing an important role in this disorder.
