摘要
1例58岁女性患者因视神经炎,给予甲泼尼龙琥珀酸钠500mg,1次/d静滴进行大剂量冲击治疗。第一次给药后次日清晨患者出现恶心、呕吐,测血压193/136mm Hg,血糖24mmol/L。立即给予口服维生素B6,肌注甲氧氯普胺,吸氧,心电监护等抢救治疗。10min后血压177/124mm Hg,心率72次/min,呼吸20次/min,2h后迅即出现呼吸、心搏骤停,面色紫绀,呼之不应,最终因抢救无效死亡。
A 60-year-old female patient with optic neuritis received an IV infusion of 500 mg methylprednisolone sodium succinate once daily. The next morning, the patients developed nausea and vomiting, and her blood pressure went up to 193/136 mm Hg and blood glucose to 24mmol/L. Methylprednisolone sodium succinate was withdrawn and she was treated with vitamin B6, metoclopramide hydrochloride, oxygen inhalation and ECG monitoring. Ten minutes afterwards, the blood pressure dropped to 177/124 mm Hg, while heart rate was 72 beats/min and respiratory rate was 20 breaths/min. However, patient＇s condition did not improve. Two hours later, the patient developed respiratory and cardiac arrest, lip and face cyanosis, no response to stimulusin. The patient died despite resuscitation attempts.
