摘要
目的：探讨慢性再生障碍性贫血患者的中医证型及影响中医证型的相关因素,从现代医学的角度指导分型,使分型更科学和准确。方法：选择本院2010年9月~2014年9月93例符合慢性再生障碍性贫血诊断标准,资料齐全的患者作为研究对象,根据其临床症状体征辨证分为3型,通过外周血常规、血清补体C3、C4及外周血T细胞亚群、NK细胞表达水平等指标进行分析,并将20例健康体检者作为对照组,分析其中医不同证型的相关因素。结果：慢性再生障碍性贫血患者WBC、Hb、PL、血清补体C3、C4及外周血T细胞亚群、NK细胞与健康对照组比较有显著差异（P〈0.05）。其中肾阳虚组WBC、Hb明显低于肾阴虚组和肾阴阳两虚组,PL明显高于肾阴虚组和肾阴阳两虚组,CD3、CD8水平低于肾阴虚组和肾阴阳两虚组,CD4水平高于肾阴虚组和肾阴阳两虚组,血清补体C3、C4含量明显低于肾阴虚组和肾阴阳两虚组,差异具有显著性（P〈0.05）。3组慢性再生障碍性贫血患者平均年龄、病程构成比例、性别构成比例比较差异无显著性（P〉0.05）;肾阳虚组、肾阴虚组及肾阴阳两虚组血清免疫球蛋白G、A、M含量及血清甲状腺激素显著低于健康对照组,TSH含量显著高于健康对照组（P〈0.05）。结论：慢性再生障碍性贫血中医证型包括肾阳虚型、肾阴虚型和肾阴阳两虚型。年龄、性别、病程等对中医证型无明显影响,而WBC、Hb、PL、C3、C4及外周血T细胞亚群、NK细胞表达水平与中医证型有一定的相关性,可作为微观辨证的参考依据。
Objective：To explore the related factors of patients with chronic aplastic anemia of TCM syndrome type and the effect of TCM syndrome types,from the point of view of modern medicine guidance,make the classification more scientific and accurate.Methods：In our hospital from September 2010 to September 2014 93 cases accorded with the diagnostic standard of chronic aplastic anemia,complete information of the patients as the research object,according to the clinical symptoms and signs of dialectic is divided into three types,through the analysis of peripheral blood,serum complement C3,C4 and peripheral blood T cell subsets,NK cell expressed level indicators,and 20 healthy subjects as control group,analysis of related factors in different syndrome type of traditional Chinese medicine.Results：Chronic aplastic anemia in patients with WBC,Hb,PL,serum complement C3,C4 and peripheral blood T cell subsets,NK cells compared with the healthy control group with significant difference（P〈0.05）.The kidney yang deficiency group WBC and Hb was significantly lower than that in kidney yin deficiency and kidney yin and yang two deficiency group,PL was significantly higher than that in kidney yin deficiency and kidney yin and yang deficiency group two,CD3,CD8 levels of less than kidney yin deficiency and kidney yin and yang deficiency group CD4 two,higher than the level of kidney yin deficiency and kidney yin and yang deficiency group two,content of serum complement C3 and C4 were significantly lower those in kidney yin deficiency and kidney yin and yang two deficiency group,the difference was significant（P〈0.05）.Mean age of patients,three groups of patients with chronic aplastic anemia course proportion,gender ratio was no significant difference（P 0.05）.Conclusion：TCM syndrome types of chronic aplastic anemia including kidney yang deficiency,kidney yin deficiency and kidney yin and yang deficiency type two.Age,sex and course of disease had no obvious effect on TCM syndrome types,whereas T cells WBC,Hb,PL,C3,C4 a
