摘要 目的探讨心脏负荷实验中超声造影对心肌微血管的损伤效应。方法 20只大白兔随机均分为4组:单纯超声触发组(A组)、静息超声触发组(B组)、负荷超声触发组0.5 ml/kg(C组)和0.05 ml/kg(D组)。心电监测有无室性期前收缩,依文思蓝(EB)视觉评分分析血管通透性,光镜观察心肌血管和细胞损伤情况。结果 B组、C组、D组EB含量均高于A组(P<0.05)。心肌表面C组、D组均见大片状出血,B组见EB。B组、C组、D组视觉评分均高于A组(P<0.05),C组、D组亦高于A组、B组(P<0.05)。A组未出现室性期前收缩,B组、C组、D组均出现室性期前收缩,组间差异均无统计学意义。B组心肌细胞间隙仅出现散在红细胞;C组、D组心肌细胞间隙出现大量红细胞,心肌细胞明显损伤。结论在心脏负荷实验中,超声造影所产生的生物学效应会对微血管造成明显损伤。 Objective To investigate the impact of contrast echocardiography on myocardial microvascular injury during cardiac stress test. Methods Twenty rabbits were randomly divided into 4 groups: simple ultrasound-triggered group(group A), resting ultrasound- triggered group(group B), stress ultrasound-triggered group with microbubbles at the dose of 0.5 ml/kg(group C) and 0.05 ml/kg( group D). ECG was applied to monitor premature ventricular contraction. Evans blue(EB) visual scores were applied to analyze the vascular permeability. Injury in blood vessels and myocardial cells were observed under the microscopy. Results In group B, C and D, EB extraction content was higher than that in group A(P0.05). Hemorrhage was seen on the surface of myocardium in group C and D, EB was observed in group B. The visual scores in group B, C and D were higher than those in group A(P 0.05). The visual scores in group C and D were also higher than those in group B(P 0.05).Premature ventricular contraction was found in group B, C and D, the differences between groups were not statistically significant.A lot of red blood cells were observed in the intercellular space in group C and D, the myocardial cells were injured obviously,while sporadic red blood cells were observed in the intercellular space in group B. Conclusion During cardiac stress test, the biological effects of contrast echocardiography would cause obvious microvascular damage.
