摘要
目的探讨实时心肌超声造影对心肌梗死后微循环灌注的定量评价。方法将21头中华小型猪随机分为心肌梗死组(MI组)和对照组,MI组结扎冠状动脉左前降支(LAD)建立急性心肌梗死模型,依据心肌梗死后不同时间点随机分为6个亚组,分别注射超声造影剂,测量参数获得造影剂心肌灌注参数(A、β、A×β)。各组实验终点动物处死后取相应心肌梗死组织,免疫组织化学方法检测微血管数量。结果与对照组比较,MI组各项灌注参数均有下降,差异具有统计学意义(P<0.05);MI 1 w组各项灌注参数指标最高,2 w、3 w和4 w组灌注参数指标均较1 w组下降(P<0.05),2 w组和3 w组各项灌注参数指标无差异(P>0.05);4 w组各项灌注参数指标下降较2 w、3 w组明显,但差异无统计学意义(P>0.05)。MI 1 w组的微血管数量增多,与3 d组相比差异有统计学意义(P<0.05),MI 4 w组微血管数量较2 w、3 w组减少,差异有统计学意义(P<0.05)。心肌造影灌注强度与病理检测的微血管密度存在正相关关系。Y(微血管密度)=1.098+0.757 6 X(心肌造影灌注强度),相关系数r=0.852,r^2=0.752(P<0.05)。结论实时超声造影可定量评估心肌梗死后微循环灌注。
Objective To study the efficacy of real-time myocardial contrast echocardiography( MCE) to myocardial micro-circulation perfusion with post-myocardial infarction. Methods Twenty-one Chinese mini-pigs were randomized into control group and therapeutic group( MI). Acute myocardial infarction model was established by ligating the left anterior descending of coronary artery The control group and therapeutic group were randomed divided into six subgroup according to the different time points after myocardial infarction and were injected ultrasound contrast agent. to obtain myocardial perfusion parameters of angiography( A,β,A×β). The myocardial infarction organization were obtained after animals sacrificed and then detected capillary number by immunohistochemical method. Results Compared with control group,all parameters in MI group were decreased and the difference was statistically significant( P〈0. 05); In MI group,the perfusion parameters were the highest in 1 w,then the perfusion parameters were decreased in 2 w,3 w and 4 w( P〈0. 05). There was no statistical difference about perfusion parameters in 2 w and 3 w( P〈0. 05); The indicators in 4w was decreased significantly compared with 2 w and 3 w,but it was not statistically significant( P〈0. 05). Compared with the 3 w group,the number of microvessel in MI 1w group was increased,and the difference was statistically significant( P〈0. 05). The number of microvessel in MI4 w group was decreased,compared with the 3 w group,and the difference was statistically significant( P〈0. 05).Myocardial imaging and pathological detection of microvascular perfusion strength density existed positive correlation. Y( microvessel density) = 1. 098 +0. 7576X( myocardial perfusion imaging intensity),correlation coefficient was 0. 852,r^2= 0. 752( P〈0. 05). Conclusion Real-time myocardial contrast echocardiography can quantitatively assess the myocardial micro-circulation perfusion with post-myocardial infarction.
