肺癌双循环血供CT灌注的初步研究 被引量:16

Evaluation of CT Perfusion Scanning in Diagnosing Lung Cancer Receiving Dual Blood Supply: A Preliminary Study

摘要 目的探讨不同病理类型及不同分化程度肺癌的双循环(肺循环及体循环)灌注特点,以期为临床诊断及预判提供指导。方法对58例肺癌患者进行前瞻性容积灌注扫描,最后经病理证实腺癌22例,鳞癌16例,小细胞癌9例,良性者11例;其中病理得到分化程度31例。采用双入口模式对其进行后处理分析分别得到病灶的肺动脉血流量(PF)、支气管动脉血流量(BF)及血流灌注指数[PI=PF/(PF+BF)]。采用LSD两两比较考察不同病理类型肺癌各灌注值之间的差异有无统计学意义,采用Spearman相关分析探究肺癌的分化程度与各灌注值之间的相关性。结果腺癌与鳞癌之间的灌注值PI差异有统计学意义(P<0.05),灌注值PF、BF差异均无统计学意义;腺癌与小细胞癌,鳞癌与小细胞癌之间灌注值PF、BF及PI差异均无统计学意义。Spearman相关分析结果显示:灌注值PI与肺癌的分化程度呈负相关(相关系数为-0.650,P<0.05),即随肺癌分化程度的降低,灌注值PI呈增高趋势。灌注值PF及BF与肺癌的分化程度无相关性。结论肺腺癌与肺鳞癌的PI值差异有统计学意义,腺癌PI值高于鳞癌;肺癌的分化程度与其灌注PI值呈负相关。双入口灌注技术对肺癌的病理分型及分化程度的临床预判具有潜在的应用价值。 Objective To investigate CT perfusion characteristics of lung cancer with different pathologic types and dif- ferentiation degrees that receives dual blood supply (pulmonary and systemic circulation) in order to provide useful guid- ance in clinical diagnosis and prediction . Methods Prospective volume perfusion scan was carried out in 58 patients with lung cancer. The pathological examination confirmed the diagnosis of adenocarcinoma in 22 eases, squamous cell carcinoma in 16 cases and small cell carcinoma in 9 cases. The pathological degree of differentiation was obtained in 31 cases. Using double-input mode to make post-processing analysis, the pulmonary flow (PF), bronchial flow (BF) , and perfusion index [ PI, PI = PF/( PF + BF) ] were thus obtained. LSD pairwise comparison was used to statistically analyze the differences in perfusion values between the different pathologic types of lung cancer; and Spearman correlation analysis was employed to evaluate the correlation between the degree of differentiation and perfusion values. Results Statistically significant differ- ence in PI existed between adenocarcinomas and squamous cell carcinomas ( P 〈 0.05 ) , while the differences in PF and BF were not significant. No significant differences in PF, BF and PI existed between adenocarcinomas and small cell carcino- mas, as well as between squamous cell carcinomas and small cell carcinomas. Spearman correlation analysis showed that perfusion value of PI bore a negative correlation with the differentiation degree of lung cancer ( correlation coefficient = 0. 650, P 〈 0.05 ), indicating that with the decreasing of differentiation the PI value was increasing. PF and BF values showed no correlation with the differentiation degree of lung cancer. Conclusion The difference in PI value between ade- nocarcinomas and squamous cell carcinomas is statistically significant, and P[ value of adenocarcinomas is higher than that of squamous cell carcinomas.
