摘要
目的探讨脑梗死后认知障碍中医证候要素,为揭示其病因病机,指导临床辨证论治提供依据。方法设计前瞻性观察方案,制定脑梗死后认知障碍中医证候观察表,通过标准化四诊信息采集,多中心协作共纳入脑梗死后认知障碍病例1504份,采用频数分析,因子分析等数理统计方法,总结脑梗死后认知障碍中医证候要素分布规律。结果因子分析结果得出6个公因子(证候要素),1 504例观察对象的证候要素中气虚占20.0%,痰占17.8%,阴虚占19.3%,血瘀占15.8%,火占11.7%,阳虚占15.4%;综合分析虚证占54.7%。结论脑梗死后认知障碍证候要素为气虚、痰、阴虚、血瘀、火、阳虚。其病位在脑,主要涉及肾、肝、脾三脏,其次为心,病性属虚实夹杂,以虚证为主。
Objective To investigate the TCM syndrome factors in patients with cognitive disorder after cerebral infarction,so as to provide evidence for etiology and pathogenesis as well as syndrome differentiation and treatment. Methods A prospective observation was designed and TCM syndrome scale in cognitive disorder after cerebral infarction was made. By gathering information of the standardized four methods of diagnosis,a total of 1 504 patients with cognitive disorder after cerebral infarction were included by multicenter collaboration. Characteristics of TCM syndrome factors distribution were summarized and discussed with frequency analysis and factor analysis. Results 6 common factors（ syndrome factors） were acquired by the factor analysis. The distribution of syndrome factors among the 1 504 patients were qi deficiency（ 20. 0%）,phlegm（ 17. 8%）,yin deficiency（ 19. 3%）,blood stasis（ 15. 8%）,fire（ 11. 7%） and yang deficiency（ 15. 4%）. The comprehensive analysis showed that deficiency syndrome accounted for 54. 7%.Conclusion The syndrome factors of cognitive disorder after cerebral infarction were qi deficiency,phlegm,yin deficiency,blood stasis,fire and yang deficiency. The location of disease was brain,mainly involved with kidney and liver,spleen. The secondary location was heart. The nature of disease is ＂intermingled deficiency and excess＂,in which deficiency syndrome is the majority part.
